For more than 50 years, Praxis Health has been the Pacific Northwest’s leading independent healthcare network, providing the highest levels of individualized patient care in Oregon.

We’re looking for a Transformation Operations Lead! This position will be a trusted operational partner to the Director of Digital Transformation, driving day to day execution across TMO initiatives enterprise wide. The role builds strong working relationships with Directors and RLCMs, brings judgment and clinical operations experience to priority and sequencing decisions, and surfaces risk early, before it becomes an escalation.

Authority follows the Decision Matrix above: Support, not Approve. Where there is a real disagreement with a Director or RLCM, the role escalates with a recommendation to the Director of Digital Transformation.

For the Oracle Health EHR implementation, project communications route through a single intake address monitored by an AI backed pipeline (Claude plus Microsoft Planner), which categorizes each item against the eight established workgroups and creates or updates the corresponding Planner task. Directors and RLCMs continue to work from Planner inside their normal cadence. This role uses the pipeline as a starting point, not a ceiling: it actively monitors workstreams and follows up with task owners rather than waiting for the pipeline to flag a problem.

The role also draws on clinical and clinical operations knowledge to proactively identify opportunities for transformative, innovative work across workgroups, beyond simply keeping existing initiatives on schedule.

Location & Schedule

  • Schedule: Monday-Friday, 8:00 AM-5:00 PM
  • Location: Flexible hybrid schedule based on events, meetings, and other business needs.

Key Responsibilities

  • Participate in driving execution across assigned workstreams: monitor progress, follow up on open items, and push sequencing and priority at the Director/RLCM level.
  • Contact any task owner directly to get status and context, using this access to build relationships and gather information across the organization. This is information gathering, not a directive to assign or redirect a task owner's work.
  • Proactively identify risk to timelines or deliverables and surface it before it becomes an escalation, not only after the intake pipeline flags it.
  • Resolve or escalate items the intake pipeline cannot cleanly route: conflicts, ambiguous or missing ownership, and cross regional dependencies.
  • Make judgment calls on cross regional trade offs where two or more Directors or RLCMs have competing priorities; where there is real disagreement, bring a recommendation to the Director of Digital Transformation rather than deciding unilaterally.
  • Attend the All Manager Huddle to represent TMO status reflected in Planner.
  • Attend the Regional Operations Committee to surface cross regional dependencies and trade offs.
  • Coordinate with the Regional Clinic Provider Lead Meeting on clinical workflow readiness items.
  • Apply clinical and clinical operations knowledge to identify opportunities for transformative or innovative workflow improvements.
  • Maintain a working understanding of the relevant workgroups for each initiative well enough to judge how an escalated item should be resolved and where priority should shift.
  • Build and maintain working relationships with Directors and RLCMs across regions.
  • Serve as a trusted advisor and thought partner to Directors and RLCMs, bringing an enterprise wide view to workstream planning and problem solving.
  • Track and report on the Proof of Value metrics defined below during the initial pilot period.

What You Bring

  • A strong operational leader who drives execution and builds influence through relationships, credibility, and follow through.
  • Experience working across multiple sites, regions, or business units where priorities legitimately conflict, and comfortable escalating with a recommendation rather than deciding unilaterally.
  • Familiarity with EHR implementations, healthcare clinic operations, or large scale system transformations is strongly preferred.
  • Clinical or clinical operations background that can be applied to spot opportunities for transformative or innovative workflow improvements, not only to track existing initiatives.
  • Comfortable working inside an existing pipeline and cadence (Planner, workstream tagging).
  • Strong written and verbal communication skills, able to represent status clearly and concisely to leaders across the organization.
  • Viewed by peers as a credible advisor and problem solver, not just a status tracker.
  • Skilled at developing others and helping Directors, RLCMs, and clinical teams achieve results through coaching, clear expectations, and shared accountability.
  • Track record of driving complex, cross-functional programs to measurable outcomes by aligning and mobilizing teams who don't report to them.
  • Translates strategic priorities into clear, sequenced execution plans, even where the path isn't fully defined.
  • Credible and confident engaging with physician, clinical, and operational leaders alike.

Required Qualifications

  • 5+ years of experience in healthcare operations, clinical operations, or clinic administration
  • At least 2 years of that experience directly supporting or leading an EHR implementation, transformation initiative, or comparable large scale system change
  • Bachelor's degree in healthcare administration, business, or a related field, or equivalent professional experience
  • Demonstrated experience working across multiple sites, regions, or business units where priorities legitimately conflict
  • Proven ability to drive execution and build influence through relationships and credibility rather than direct authority
  • Strong written and verbal communication skills, with experience presenting status and recommendations to senior leaders
  • Working proficiency with project or task tracking tools such as Microsoft Planner or a comparable platform

Preferred Qualifications

  • Direct experience with Oracle Health (Cerner) or another major EHR platform implementation
  • Clinical or clinical operations background (RN, MA, or similar) that can be applied to identifying workflow improvement opportunities
  • PMP or a similar project or program management certification
  • Experience in a multi site or multi region healthcare network spanning more than one state
  • Familiarity with formal change management frameworks
