To learn more, visit www.habitathealth.com.\nRole Scope: The Functional Wellness Supervisor leads and directly oversees the rehab team which includes Physical Therapists (PT), Occupational Therapists (OT), Aides, and Home Care Coordinators at the PACE center. This role ensures that therapy interventions are high-quality, participant-centered care that supports independence, safety, and quality of life. The Functional Wellness Supervisor facilitates functional wellness programming, performance improvement, and cross-departmental collaboration. This role is responsible for the day-to-day team supervision, training and competency oversight, and regulatory compliance for the rehab team working in close partnership with Center leadership and the interdisciplinary team. \nCore Responsibilities & Expectations for the Role \nLead rehabilitation and functional wellness services at the Center to support independence, safety, and quality of life. \nProvide oversight for Occupational Therapy, Physical Therapy, and integrated functional wellness programming. \nAlign clinical operations with PACE regulations, CMS requirements, state licensure standards, and organizational goals. \nPartner with the interdisciplinary team and center leadership to ensure clinically appropriate, evidence-based, participant-centered interventions and care plans. \nManage staffing, scheduling, and workflows; coach performance and ensure reliable service coverage. \nMonitor key performance indicators (utilization, functional outcomes, and participant satisfaction) and drive quality improvement through audits and corrective actions. \nBuild a data-driven practice by standardizing functional outcome measures, strengthening documentation of measurable progress, and creating reporting/dashboards that translate outcomes into operational and clinical insight. \nSupport efforts to reduce preventable hospitalizations through proactive functional interventions and coordinated care. \nLead fall prevention and recovery initiatives: screening and risk assessment, individualized and group interventions (balance/strength training, environmental modification, caregiver education), incident analysis/root cause review, and coordination with nursing and medical providers. \nChampion innovation and continuous learning by integrating current geriatric rehabilitation research, piloting new programs/tools, evaluating impact, and scaling best practices across PACE centers. \nEmphasize functional wellness to reduce acute utilization, recovery after hospitalization/illness and long-term aging-in-place. \nRequired Qualifications: \nUnrestricted Occupational or Physical Therapy license in the state of practice (or eligible for licensure) with a bachelor’s degree from accredited program. \nTwo to four years of experience with at least one of those leading people including performance coaching, scheduling and workflow management. \nDemonstrated experience in geriatric rehabilitation, functional assessments, and evidence-based interventions. \nStrong knowledge of PACE regulations, CMS requirements, and state licensure standards. \nExperience developing or overseeing functional wellness programs, fall-prevention initiatives, and aging-in-place strategies. \nAbility to interpret and act on clinical and operational data, including functional outcome measures and KPIs driving quality improvement, innovation, and continuous learning. \nSkilled in interdisciplinary collaboration and participant-centered care planning. \nExcellent communication skills with a commitment to creating a welcoming, inclusive, and joyful center environment. \nAligns with our purpose and our values, and is excited about living those out in daily practice \nAbility to thrive in a fast-paced, evolving environment with comfort navigating ambiguity, adapting quickly, and contributing to continuous improvement \nStrong learning and growth mindset, including seeking feedback, engaging in healthy debate and using data and curiosity to inform decisions. \nActs with integrity and ownership, considering the broader organizational impact, doing the right thing, and following through to deliver results. \nNice to have: \nAdvanced degree in PT or OT. \nEssential Functions & Physical Requirements \nThe following statements describe the general nature of work and physical expectations common across roles at Habitat Health. This position is also eligible for a target annual bonus and may be eligible for additional incentive compensation, as applicable. \nThe final compensation offered will be determined based on factors including, but not limited to, the candidate's qualifications, skills, experience, education, certifications, geographical location, and business needs. \nIn addition, this position is eligible for a comprehensive benefits package, including medical, dental, and vision insurance; paid parental leave, short- and long-term disability coverage; life insurance; flexible spending accounts; a 401(k) savings plan that includes a match; paid time off; and company paid holidays. \nAt-Will Employment \nEmployment with Habitat Health is at-will.
