Habitat Health empowers older adults to experience more good days in their homes and communities. Through the Program of All-Inclusive Care for the Elderly (PACE), we provide comprehensive medical care along with support for daily needs such as meals, transportation, and in-home assistance. We deliver coordinated clinical and social care in our centers and directly in participants’ homes, creating a fully integrated experience that brings peace of mind and a true sense of belonging. As we expand our scalable, affordable PACE model to meet the growing and complex needs of aging populations, our mission-driven care teams continue to help participants live well on their own terms.
Habitat Health is supported by leading healthcare organizations and investors including New Enterprise Associates, Kaiser Permanente, and Town Hall Ventures. We are entering a period of significant growth and are looking for exceptional teammates to help us scale a better model of care for older adults. To learn more, visit www.habitathealth.com.

Role Scope
Habitat Health’s Product team is focused on using AI, technology, and disciplined workflow design to turn how the company works into scalable, measurable capability. We’re looking for a smart, driven person to help the team do more - someone who takes ambiguous problems off our plate and turns them into shipped, adopted, measurable results.
This is a broad role, and deliberately so. In a given week you might map how an operational process actually works today, build a scrappy automation that saves the team hours, sit in a center watching the real workflow, pull data to settle a debate, and then own the rollout that makes a new tool actually stick. We care far more about horsepower, curiosity, and ownership than about a specific background - if you’re the kind of person who sees a manual, repetitive process and can’t help but fix it, you’ll fit here.
You’ll work directly with product, engineering, data and functional owners across clinical, operational, enrollment, and other enterprise teams. This is a high-trust, high-exposure seat close to how the company runs.
What You’ll Do
Turning ambiguity into working systems
- Jump into whatever the product team needs most - research, analysis, process mapping, AI deployment, docs, coordination, a quick prototype - and bring structure to fast-moving, multi-stakeholder work.
- Break big fuzzy asks from stakeholders into clear, actionable plans, and keep the relevant people aligned as things move.
- Hunt down manual, repetitive, or error-prone processes and eliminate them - with off-the-shelf tools, no-code builders, light scripting, AI, or a clean spec handed to engineering.
- Use AI productivity tools (Claude, ChatGPT, and the like) as a daily default to accelerate drafting, analysis, and operational workflows.
- Work with the Product, Technical, and functional leads to build systems and reporting - templates, dashboards, checklists, simple integrations - that makes good work repeatable.
Get results on the ground
- Travel to Habitat centers to see how the work really happens, build trust with front-line teams, and become the face of the tools and workflows we roll out.
- Support adoption end to end: onboarding, training, follow-through, and iteration until something is actually used, not just launched.
- Chase measurable outcomes - adoption, throughput, cycle time, quality, administrative burden - and let the data drive what you do next.
What We’re Looking For
We’re optimizing for raw ability and drive over years of experience. If you can point to times you figured hard things out and got them done - in a job, a startup, a research project, a side project, or school - we want to hear from you.
- Smart, and a fast learner who gets up to speed on unfamiliar domains, tools, and systems quickly.
- A genuine love of efficiency and automation - you instinctively look for the repetitive task and the tool that could kill it.
- High ownership and strong bias toward action: as ready to write the training doc and sit with a front-line user as to design the process.
- Comfortable with ambiguity and an early-stage environment where the playbook doesn’t exist yet - you’ll help write it, improvise, and adjust as things change.
- Analytical instincts: you can define what “better” means, pull and read the data, and use it to prioritize.
- Strong written and verbal communication - able to summarize the takeaway for a leader and get into the tactical weeds with an execution team.
- Hands-on with everyday tools (spreadsheets, docs, dashboards) and eager to pick up whatever the job needs; light scripting, SQL, or no-code experience is a plus, not a requirement.
- Willingness to work in a hybrid environment with travel to Habitat Health centers as needed.
Nice to Have
- Exposure to healthcare, value-based care, provider or payer operations, Medicare, Medicaid, PACE, or complex care delivery - or genuine curiosity about it.
- Experience driving adoption of new tools or workflows across distributed, front-line teams.
- Familiarity with Salesforce, Epic, or healthcare data (EHR, referral, or enrollment data).
- A track record of teaching yourself the tool you needed to get something done.
