Responsibilities include screening of clinical information on referrals to ensure that appropriate orders and discharge information is received. The Intake Coordinator will work with the Billing Dept to provide any clinical information required to obtain initial authorizations and will maintain communication regarding tracking of insurance authorizations about to expire. The Intake Coordinator will be responsible for obtaining ongoing insurance authorizations as needed. The Intake Coordinator will have an active role in our Tele-health Program. Responsibilities also include assisting in answering and directing calls coming into the main home care line, assisting in triaging patient and family calls, and working closely with the Scheduler regarding scheduling of patient visits.\nPosition Responsibilities Include:\nAssist in answering and directing calls that come into the main home care line.\nTriages patient inquiries and assists Clinical Managers and Scheduling Coordinator in communication and scheduling of staff as needed.\nResponsible for obtaining referral information from all referral sources- direct referral source phone calls, 4Next and the Allscripts referral management system utilizing e-fax.\nOngoing communication with Home Care Liaison Team to ensure notification of all referrals is obtained.\nPerforms Intake and Referral functions including: capturing accurate patient information, data entry of referral information, relevant medical history and current orders into EMR, including coding of primary home care diagnosis.\nObtains initial F2F documents as well as patient discharge paperwork from referral source according to agency and regulatory standards.\nVerify patient’s primary insurance, and communicate this information to the Billing Dept to request initial authorizations as needed. Work closely with the Billing Dept to provide any clinical information that may be required to obtain initial authorization.\nTrack insurance authorizations and provide ongoing clinical information required to obtain ongoing authorizations as required and communicate this information to the clinical staff.\nNotifying Clinical Managers and Scheduling Coordinator of all referrals, including any pertinent clinical issues that may require assignment to a clinician with specialty. Responsible to communicate any changes in referral status to Clinical Managers and Scheduling Coordinator.\nAssists agency management and office staff in the maintenance of the patient medical record including EMR and paper files as appropriate and per agency policy/protocol.\nNotify Home Care Liaisons of any active patients that have been hospitalized.\nResponsible for enrolling all identified patients in Telehealth. This includes all aspects of enrollment- Completion of enrollment information to be sent to Telehealth monitoring company, ensuring telehealth machine is available for the clinician to pick up with all required documentation included and communicating to Clinical Managers and Scheduling Coordinator when Telehealth visit is to be scheduled.\nDaily monitoring of Telehealth readings and ongoing communication to Clinical Managers and Clinicians regarding clinical concerns. This may also include communication to Scheduling Coordinator if it is determined clinical visit needs to be scheduled.\nProvides other related duties as required or directed by home health administration.\nMaintains compliance with policies, procedures, and regulatory matters.\nParticipates in personal and professional growth and development including staff meetings and in-service education.\nPromotes and maintains an agency environment that is in compliance with federal, state, and local regulatory agencies.\nCommunicates with patients, families, and other health professionals in a manner that conveys respect, caring, and sensitivity.\nProvides answering service with on call details during closed office hours\nResponsible to rotate to cover Intake Responsibilities on holidays as required.\nThe above covers the most significant responsibilities of the position.
