The program provides direct services to caregivers, offering supportive counseling, information and assistance with benefits/entitlements, support groups, caregiver training; The program conducts extensive outreach and provides limited respite and supplemental assistance services.\nUnder the direction of the director, the Outreach Specialist is an integral part of the team, creating and implementing an outreach plan to engage prospective clients with the program; especially underserved communities. Engaging and cultivating relationships with care givers and care receivers as well as professionals. Along with Caregiver Service Program staff ensure that information and resources are widely disseminated; and develop collaborations with community partners, religious institutions and health care providers, hospitals, Elected Officials and Business EAP contacts to generate expanded services for caregivers. The position also will require leading educational presentations and representing HMH at community tabling events.\nResponsibilities\nAssist Director in developing outreach material, implementing outreach plans, and analyzing data from surveys of constituents (e.g. caregivers, care receivers, referral sources).\nProvides Community Outreach and Partnerships/Collaborations.\nWork with program staff to create and deliver workshops, on caregiver services, eligibility and HMH Caregiver Program.\nIdentify resources (e.g. training videos, tip sheets) that can be used for caregiver training or for social media platforms. \nMaintain records of public information contacts and educ/ outreach workshops. \nUpdate the agency website with training resources for caregivers on the caregiver services page.\nConnect interested clients with contact information and if indicated, collect consent from potential clients.\nWork with Director and team to maintain and expand existing community partnerships by being active and visible in the community and working closely with other professional, business, civic and private organizations as indicated.\nAssist with the development of content for social media platforms to keep Manhattan 1 catchment area caregivers, referral sources, and stakeholders informed of Program activities and resources. \nCreate Partnerships to enhance service provision as well as breadth of social media platforms. \nRepresent HMH in outreach, planning and execution of special events, including workshops, health fairs, community events,\nDevelop strategies to reach isolated or difficult to find caregivers.\nMay be required to work evenings or weekend days for outreach events as needed.\nOther duties as determined by Director and required by funding source.\nMinimum Qualifications\nMSW/BSW or related degree \nPreferably 1 year experience with older adults desirable \nStrong commitment to the mission of the Caregiver Services Program \nExperience facilitating workshops and providing outreach preferred\nNonjudgmental, socially perceptive, empathic and a good listener \nSensitivity to understanding the needs of caregivers and older adults. \nKnowledge of client engagement, assessment, and the biological, psychological, and social aspects of aging and the impact of disabilities and illness on aging and the family \nHighly organized and detail oriented; able to plan, manage and successfully complete multiple concurrent projects, frequently with short deadlines \nAbility to handle complex and difficult situations in a professional manner. Demonstrated ability to conduct oneself in a calm and professional demeanor when dealing with the public and/or with difficult situations \nAbility to develop collaborative relationships with professional staff and community groups \nExcellent verbal and written communication skills including creation of marketing and outreach materials \nExcellent understanding and skills related to social media including snapchat, Instagram, Facebook, and website content \nEngaging and Strong interpersonal skills demonstrative of empathy and support \nWillingness to make community visits and take initiative\nStrong written and verbal communication \nSolid Microsoft Office experience \nUnderstanding and appreciation of the role of race, religion, ethnicity, sexual preference and individual values as it relates to serving clients and their families.
