You will play a key role in supporting our Care Professionals, maintaining high standards of care, and ensuring our clients receive compassionate, person-centred support in their own homes.\nThe Role\nSupport care consultations, service reviews and completing client support plans.\nConduct Care Professional support visits and client Quality Assurance reviews.\nCreate, update and audit client care plans and assist with digital care planning.\nMaintain accurate client and Care Professional records on Home Instead software and People Planner.• Conduct client and Care Professional introductions.\nBuild and maintain positive relationships with potential and existing clients, their families and other professionals involved in their care.\nCarry out client reviews and action these accordingly.\nSupport and mentor Care Professionals.\nProvide support to the Client Experience team.\nMaintain regular contact with clients and Care Professionals.\nAny other duties as directed by your line manager.\nComply with Home Instead’s Equality, Diversity and Equal Opportunities Policy, ensure in respect of employment and service delivery.\nCarry out any other duties deemed necessary for the successful operation of the business.\nParticipate in out of hours on-call cover during the weekdays once a week (included in contract), and weekends one a month (additional payment).\n • Level 3 NVQ in Health and Social Care or equivalent preferable.\n• Experience in the care sector delivering a wide range of care services\nPassion for delivering the highest quality care to our clients and helping them to live independently and happily in their own homes.\nKnowledge and understanding of legislation and regulations specific to Health and Social Care.\nGood communication skills with the ability to build rapport quickly.\nMust be confident to use care management technology including providing support and training to Care Professionals\n Must have full driving license and means of transport to carry out visits within the territory.\nIf you have a passion for caring and the motivation to help us achieve our ambitions, we would love to hear from you.\nWe encourage applications from all sections of the community as we want to reflect the neighbourhoods in which we work.
