Assists, observes, and reports patient changes to licensed caregiver and maintains accurate medical record documentation relative to the care delivered.\nESSENTIAL FUNCTIONS\nProvides and collaborates direct patient care services under the direct supervisor of a licensed caregiver. Responsibilities may include, but are not limited to personal care, activities of daily living, vital signs, intake and output, ambulation, interventions, and implementation of selected portions of the patient care plan as directed under supervisor of nurse.\nAssists, observes and reports changes in patient condition to licensed caregiver and maintains accurate medical record documentation in relation to the care delivered.\nAddresses and/or directs questions or concerns from patients, families, caregivers, or health care team members.\nPerforms other duties as assigned.\nEDUCATION\nHigh School Diploma or GED Required\nEXPERIENCE\nOther Completion of Block 1 in Nursing OR Medical Assistant (MA) OR Nursing Assistant (NA) OR Emergency Medical Technician (EMT) OR Behavioral Health Technician (BHT) OR 6 months of experience in the healthcare setting within the last year Required\n6 months in acute care experience.
