Over the next three years, HousingPlus’ programs will continue to grow, along with its commitment to serving vulnerable populations. \nPlease consider joining a team committed to building brighter futures for all!\nPlease visit our website for more information regarding our organization: www.housingplusnyc.org.\nPlease do not call or email HousingPlus regarding the status of your application\n Position Summary:\nThe case manager will work in our congregate or scattered site supportive housing program(s) in Brooklyn for women and women with children who have experienced chronic homelessness, mental illness substance use disorder or a disabling medical condition. The Case Manager will carry a caseload of individuals, families, or a combination of both and will deliver services to support the goals of housing stability, family stabilization, independent living, and financial security.\n Duties/Responsibilities:\nConduct initial intakes and assessments and annual assessments as required.\nDevelop initial Individual Service Plan (ISP) including relapse prevention plans with all caseload tenants. Conduct quarterly ISP reviews and annual ISP updates with caseload tenants as required.\nProvide case management, counseling sessions and support services as required.\nRefer tenants to other agencies, community service providers and resources as needed. Collaborate with providers to ensure compliance.\nEscort tenants to appointments as needed.\nAssist families and/or individuals with achieving goals of self-sufficiency and independent living.\nWork with individual and/or the entire family to improve family functioning.\nMaintain/update case notes and other data in AWARDS database as required. Document all interactions with tenants in AWARDS within 72 hours.\nEnsure progress notes provide accurate and detailed notes on needs, challenges and barriers, and progress and successes for all family members.\nMaintain tenant case files that are organized and easy to navigate.\nEnsure case files include AWARDS and supporting documentation for all family members as required.\nMaintain housing goal of zero rental arrears for each family and/or individuals. • Provide emergency and crisis interventions.\nConduct monthly home visits as required for entire caseload.\nAttend program meetings, agency-wide meetings, and staff training.\nPerform program objectives in accordance with HousingPlus and government funder policies and procedures.\nOther tasks as assigned.\nREQUIREMENTS:\n Required Skills/Abilities: \nAbility to work onsite 5 days per week\nEnglish/Spanish speaking preferred, not required.\nExperience working with veterans preferred, not required.\nAbility to work as a team member.\nFamiliarity with Human Resource Administration (HRA) policy and procedures.\nExcellent organizational, interpersonal communication, and writing skills.\nProficient computer skills including Microsoft applications (Outlook, Work, Excel, PowerPoint) and Google Suite. Must possess the ability to format and work within spreadsheets and Word Documents.
