This money is available for tuition loan reimbursement, tuition costs, and text books.\nOverview:\nThe Housing Works Emergency & Transitional Housing Program (ETH) provides time limited transitional housing and support services to individuals reentering the community following periods of incarceration or other justice system involvement. Program participants engage in a variety of onsite services, including case management, vocational training, housing advocacy and mental health support that address barriers to accessing housing in the community and help build skills that participants can use to achieve and maintain stable functioning in the community.\nReporting to the Assistant Director, the Intake Coordinator plays an integral role managing program referrals and coordinating admission. These activities include managing the program waitlist, collaborating with internal and external referral partners, screening and reviewing referrals for eligibility and program site placement, data entry and record keeping and managing the initial admission and enrollment activities. This is an ideal position for professionals with social services and data entry experience seeking to further develop skills and experience in the reentry and housing sector. They will have the opportunity to work along side like-minded professionals committed to improving outcomes for justice-involved individuals.\nResponsibilities:\nPrimary:\nClosely review and monitor program census and submit daily vacancy reports.\nReview and screen referrals received for the ETH Program and determine eligibility and appropriate program site placement.\nAssist with gathering additional supporting documentation or medical records when necessary to determine eligibility.\nCoordinate with the interdisciplinary care teams when necessary for further evaluation and screening of referrals received to determine level of care needs.\nManage the program waitlist utilizing the electronic records systems in place.\nCoordinate with the referral partners or candidate for admission when there is a vacancy.\nFacilitate recurring meetings with key stakeholders and referral agencies to provide status updates for submitted referrals.\nComplete daily, weekly, monthly and quarterly reports timely and accurately.\nMonitor and report referral and placement outcomes.\nEnsure data entry is accurate and timely.\nCreate new charts in the electronic medical records system for all newly arriving clients.\nPromote Housing Works’ core values in day to day activities.\nPerforms other tasks as assigned by supervisor\nSecondary:\nParticipate in agency advocacy events.\nWork collaboratively across teams and divisions.\nActively participate in supervision and professional development trainings.\nOther functions that may be required with this position include:\nResponsiveness to emails, texts and phone calls.\nRequirements:\nBachelor’s Degree required in social services, psychology, social work or other related field or Associate’s Degree with 2+ years of social services or data entry experience.\nThe following are qualifications and requirements of the position:\nData entry experience.\nExperience working in a heath care or non-profit setting.\nStrong written documentation and communication skills\nGood problem-solving skills and ability to work autonomously.\nProficiency in Microsoft Office Suite, particularly Excel, required.\nExperience utilizing EMR systems.\nThe following additional competencies are viewed as important to success in this position:\nPreferably bi-lingual in Spanish.\nExcellent written and oral communication skills.\nExcellent or proficient technical skills to perform tasks related to Microsoft Office, email and case management systems.\nAbility to work independently and collaboratively with the team.\nDependability and reliability to arrive and depart timely for scheduled shift.\nAbility to frequently travel within the boroughs via public transportation.\nJob candidates should be aware that scammers may pose as employers and create fake job postings in order to extract personal information from individuals for financial gain.
