Provide recommendations and follow-up if necessary.\nFacilitate coordination and collaboration with other community-based organizations and partners.\nEnsure program integration with other agency services.\nDevelop a steering committee and coordinate quarterly meetings.\nAssist in the coordination of all data gathering, analysis, and program reporting.\nPrepare SAMHSA Progress Reports.\nCollect monthly reports from the team, monitor service delivery trends, identify areas for improvement and make a corrective action plan.\nOversee data entry into the system and monitor project progress.\nRepresent organization in different forums.\nSupport the work team in everything necessary for the continuity of services.\nSupport the evaluator in the collection of information.\nReport project progress or challenges to the project director.\nRequirements:\nBA or master’s degree in related field\nExperience with direction or coordination of programs directed to people that have experienced homelessness, substance use, and/or co-occurring disorders in community settings.\nExperience with supervision and disciplinary management.\nExperience with Evidence Based Practices / Interventions\nKnowledge about MAT/HIV/AIDS, Hepatitis-C, and STDs in the Puerto Rican context\nAble to work closely with others in outreach team and office setting (team player)\nProficient and efficient with multiple computer software programs\nMeticulous at maintaining records and confidentiality protocols\nOutreach experience\nUp to date driver’s license\nJob candidates should be aware that scammers may pose as employers and create fake job postings in order to extract personal information from individuals for financial gain.
