Rather than focusing on archaic outdated design, we strive to consistently question the “status-quo” and create new and more innovative ways to help aging adults and adults with disabilities maintain their quality of life.\nWith over 23 programs, we offer multiple ways to aid seniors maintain their health, well-being, independence and participation in the community, fulfilling our mission.\nThe primary responsibility of the Clinical Supervisor is to provide clinical supervision of Community Living Fund (CLF) Intensive Care Management (ICM) staff including Master’s level Social Workers, Occupational Therapists, Money Managers, and others.This is a temporary, per diem role designed to provide operational support on an as-needed basis. Hours are variable, up to a maximum of 19 hours per week, based on department needs.\nKey duties and responsibilities include, but are not necessarily limited to the following:\nResponsible for the clinical supervision, training, and evaluation of the Care Management staff participating in the Community Living Fund in collaboration with Partner agencies.\n\nParticipates in the hiring and termination of CLF Care Management staff.\n\nFacilitates the day-to-day operations, ensuring appropriate systems are in place.\n\nMonitors program and staff compliance with all regulatory requirements, such as San Francisco Department of Aging and Adult Services/San Francisco Human Services Agency and Partner organizations.\n\nEnsures high quality, timely care, and service delivery.\n\nImplements and monitors multiple systems, including records, clinical documentation and personnel. Ensures that documentation is compliant with regulations, policy and procedure manual, and is complete and accurate.\n\nOrganizes and conducts regular care planning meetings and supervision.\n\nFacilitates the orientation and training of all new staff, including joint home visits, supervisory meetings, and documentation review.\n\nResponsible for assessing the educational needs of staff, developing and coordinating an education and training plan that assures a high level of skill and competency in the understanding of case management, community resources, and family systems.\n\nEstablishes and implements Quality Assurance mechanisms to measure and maintain high standards of care and programming, including regular chart reviews and peer review.\n\nDevelops and maintains collaborative working relationships and cooperative arrangements with community individuals, groups and organizations; takes advantage of opportunities to educate the public and the community regarding CLF services.\n\nResponsible for remaining current with changes in San Francisco community resources, Medi-Cal and Medicare regulations and procedures that will impact the practice and direction of the program; acts as a resource to CLF and IOA staff.\n\nResponsible for ensuring CLF purchase of service funds are used as a last resort and all community resources and referrals are made prior to the authorization of CLF funds; verifies CLF funds are used to prevent institutionalization when there are gaps in client care.\n\nProvides back-up coverage for clinical staff.\n\nWorks with local schools to secure and to supervise social work interns.\n\nIn collaboration with the Director of the Community Living Fund, participates in establishing annual goals and objectives relevant to the needs of the program and in planning, developing and evaluating policies and procedures; actively involves staff in the process.\n\nActively participates in and encourages actions that promote good public relations with clients, their families, and the community.\n\nResponsible for expanding personal knowledge, skill and attitudes specific to care management, family systems, geriatrics and gerontology, addiction and other relevant management or health care issues.\n\nProvides Clinical Supervision for Master’s level social workers.\n\nMay direct and monitor additional smaller programs to adhere to contract guidelines and acceptable clinical standards of practice.\n\nAccepts and performs other related duties and responsibilities as required.\n\nREQUIRED QUALIFICATIONS:\nMaster’s degree in Social Work (MSW) or a Master’s degree in a comparable or related social services discipline.\n\nFive years of experience working directly with disabled adults and/or the frail elderly.\n\nDemonstrates ability to supervise intensive care management staff who work with adults with disabilities at risk of hospitalization, which present with complex medical conditions, mental health diagnoses, substance abuse, as well as physical rehabilitation needs.\n\nDemonstrates understanding of behavioral health services and behavioral health interventions.\n\nProven previous training of staff as it relates to the population served through CLF.\n\nDemonstrates understanding of the health and psychosocial needs of this population.\n\nDisplays knowledge of community resources available for this population.\n\nDemonstrates ability and willingness to take on program development projects.\n\nProven ability and willingness to provide leadership amongst peers and mentor junior staff.\n\nCompensation\nRange: $57.25 -
