Multipurpose Senior Services Program (MSSP) provides interdisciplinary care management services to individuals at risk of being institutionalization or currently institutionalized that can transition home or “home like setting” providing services are in place to help them remain safe in the community.ESSENTIAL JOB FUNCTIOINS:\nResponsible for the clinical supervision, training and evaluation of Registered Nurse, Social Work Care Management and Admin Staff working in MSSP.\nParticipates in the hiring, yearly performance evaluations and termination of MSSP Care Management staff.\nFacilitates and oversees day-to-day operations; insuring that appropriate systems are in place.\nPerforms regular audits of cases\nEnsures program and staff compliance with all regulatory requirements as set forth by California Department of Aging.\nInsures high quality, timely care and service delivery\nImplements and monitors multiple systems, including records, clinical documentation, and personnel. Ensures that documentation is compliant with regulations, policy and procedures.\nOrganizes, conducts care plan conferences and individual supervision as needed\nFacilitates the orientation and training of all new staff, including joint home or virtual visits, supervisory meetings, and documentation review.\nResponsible for assessing the educational needs of staff, developing and coordinating an education and training plan that assures a high level of skill and competency in the understanding of case management and community resources.\nIn collaboration with Leadership and MSW Clinical Supervisors, lead the development and implementation of MSSP curriculum for all Care Management staff\nDevelops and maintains collaborative working relationships with community individuals, HHA’s, PCA’s, groups and organizations; takes advantage of opportunities to educate the public about MSSP services.\nResponsible for remaining current with changes in community resources, Medi-Cal and Medicare regulations and procedures that will impact the practice and direction of the program; acts as a resource for MSSP staff.\nProvides back-up coverage for clinical staff which could include home and/or virtual visits.\nIn collaboration with the leadership, participates in establishing monthly, quarterly, and annual goals and objectives relevant to the needs of MSSP.\nIn collaboration with Program Manager and Clinical Supervisor, conducts annual review and revision of program policies and procedures.\nResponsible for expanding personal knowledge, understanding of care management, family systems, geriatrics, adolescent, relevant heath care issues.\nUnderstands and applies the regulatory and procedural requirements of MSSP as well as the policies and procedures of the Institute on Aging.\nProvides mentorship, consultation, and training for all new MSSP CM staff and interns.\nAssumes responsibility for some of the program’s most challenging clients.\nParticipates in program leadership meetings and takes on program development projects as assigned.\nActs as liaison between MSSP, CDA, and providers.\nMay direct and monitor additional smaller programs to adhere to contract guidelines and acceptable clinical standards of practice.\nTravel when necessary\nAll other reasonably related responsibilities as assigned.\nEDUCATION: RN, BSN. Preferred: MSN, Master’s degree in nursing\nBACKGROUND AND EXPERIENCE:\n3-5 years’ experience working with frail elderly, adolescents, disabled adults and children, acute care and/or Home Health required.\nSupervisory experience (preferred)\nExperience with and understanding of the medical and psychosocial problems\nExceptional communication, presentation skills, and time management.\nDetail oriented and well organized.\nDemonstrates case management skills and experience in the community health care delivery system.\nDemonstrates ability to supervise care management staff and provide coaching / mentoring to staff.\nDemonstrates ability and willingness to take on program development projects.\nGood problem-solving skills and the ability to prioritize and multiple tasks.\nComputer literacy required (Excel, Word).\nAbility to navigate multiple platforms and EMR’s.\nPHYSICAL & LANGUAGE REQUIREMENTS:\nAbility to lift, transfer, push/pull, maneuver, and reposition up to 25 lbs\nAbility to reach, bend, and walk\nFive finger manipulation\nAbility to comprehend complex written materials and verbal instructions\nAbility to speak and write English clearly\nCompensation\nRange: $123,000 -$128,000/annual\n\nThis amount is not necessarily reflective of actual compensation that may be earned, nor a promise of any specific pay for any specific employee, which is always dependent on actual experience, education and other factors.\nThis range does not include any additional equity, benefits, or other non-monetary compensation which may be included.\nBeware of Hiring Scams\nWe are aware that some third parties have reposted our job listings in an attempt to scam applicants.
