Our work focuses on inclusion and building communities where everyone has the opportunity to thrive and fully participate in everything society has to offer.\nThe Program Coordinator supports individuals with disabilities by teaching, coaching, and modeling the skills needed to participate fully in family, community, cultural, and civic activities. Our team has access to a competitive benefits package comprised of medical, dental, vision, paid time off, 401K, career advancement, tuition reimbursement, wellness programs, and more!\nKey Responsibilities:\nSupport individuals in accessing community resources, building life skills, and increasing participation in meaningful daily activities \nProvide training, guidance, and direct support to enhance personal competencies and promote social integration \nDeliver direct care support based on program needs \nProvide transportation for individuals as needed \nPerform general housekeeping tasks and maintain cleanliness of the home and property \nMaintain inventory of household equipment and furnishings in accordance with agency standards \nMentor and train new employees, including orientation support and documentation coaching \nAssist with schedule development, maintenance, and coordination of daily activities \nReview, validate, and approve service documentation and daily notes to ensure accuracy and compliance with individual outcomes \nSupport supervisors with timely and accurate billing processes \nCoordinate and attended medical appointments, ensuring follow-through on physician orders \nComplete all required documentation within established timeframes. \nFollow all medication administration and monitoring procedures, including required documentation \nMaintain organized record-keeping systems, including Person-Centered Planning, treatment documentation, and discharge planning \nCollaborate with families, support professionals, and interdisciplinary team members to ensure cohesive service delivery \nCoordinate assessments, support plans, and implementation of individualized services \nParticipate in on-call rotation as required \nJob Details:\nFull time, non-exempt position \n$22.50 per hour \nMinimum Requirements:\nOne of the following: \nHigh school diploma or equivalent with three months of related work experience \nAssociate or bachelor's degree with related work experience \nTwenty-one years of age or older \nProgram administrator certification or ability to obtain certification within six months \nEffective communication and proficient computer skills \nValid driver’s license with daily access to a privately maintained and insured vehicle \nSuccessful completion of the pre-employment process, including clearances and verifications \nKeystone Human Services prohibits discrimination against qualified individuals based on their status as protected veterans or individuals with disabilities and prohibits discrimination against all individuals based on any category protected by applicable federal, state, or local law.\nDisclaimer: The preceding job description has been designed to indicate the general nature and level of work performed by employees within this job.
