Train and support host home providers. Interact with department director to ensure quality of life and upward development of participants.\n Duties/Responsibilities:\nMonitor and implement individual residential programming for participants on caseload.\nEnsure participants’ health and safety needs are met.\nMonitor participants’ medical needs.\nSupport participants and host home providers (HHPs) with participants’ finances.\nPrepare for and attend Service Plan (SP)/Interdisciplinary Team (IDT) meetings.\nDevelop and train HHP to implement participants’ ISSP programs according to identified SP outcomes.\nMonitor Individual Service and Support Plans (ISSPs) tracking monthly and revise programs as needed.\nMonitor participant main and working files and complete follow-up.\nEnsure program adheres to regulatory agency guidelines.\nReview participant incident reports, identify trends and provide necessary follow-up.\nEnsure Critical Incident Reports are completed and filed within designated timelines and ensure follow-up as needed in accordance with regulations.\nComplete and submit Human Rights Committee (HRC) paperwork and follow-up on recommendations according to established deadlines.\nAssist and participate in quality assurance procedures and policy development to ensure compliance with governing agencies.\nEstablish and maintain written and verbal communication with governing agencies, families, guardians, HHPs and other professionals.\nMaintain benefit eligibility, i.e., Social Security, Medicaid, Medicare, etc.).\nConduct and document monthly host home visits and provide training and follow-up as needed.\nFacilitate and attend HHP functions as needed.\nParticipate in monthly residential and adult services meeting.\nDevelop host home placements for participants.\nMaintain HHP contract files.\nEnsure HHPs are in compliance with contract.\nOn-call 24-hours on a rotating basis.\nMaintain regular and punctual attendance.\nOther duties as assigned.\nMinimum Training Requirements\nAll trainings deemed mandatory by Laradon\nA minimum of 24 hours of approved training annually\nOrientation\nFirst Aid/CPR\nMedication Certification\nCrisis Intervention training\n20 hours of on-the-job training\nMinimum Position Requirements\nBachelor’s degree in Human Services or related field.\nFive years direct experience may be substituted for degree requirements.\nExcellent verbal and written communication and problem-solving skills.\nGood customer service and people skills.\nValid driving license, good driving record, and reliable personal transportation.\nMust be able to lift, push and pull 25 pounds\nMobility, bending, sitting, stooping.\nStamina to be on feet and active for full workday.\nWe proudly support our employees by providing the following benefits for this position:\nMedical/Dental/Vision Insurance/FSA\n401(k) + 4% Match\n11 Paid Holidays and 16 days of Paid Time Off accrued per year.\nFAMLI Leave\nBilingual Pay Differential\nTuition and Certification Reimbursement\nStudent Loan Assistance\nLaradon is a qualifying employer for the federal Public Service Loan Forgiveness (PSLF) program\nEmployee Assistance Program\nVoluntary Long Term Disability, Life, Cancer, Accident, and Pet Insurance\nEcoPass: Unlimited rides on RTD buses and trains for full-time employees\nReferral Bonuses (get paid to work with friends!)\nCommittee participation opportunities (DEIB/A, Safety)\nDevelopment, job growth, and internal training\nThe opportunity to work in a dynamic, supportive, and rewarding environment alongside other Laradon staff -
