Be part of an extraordinary life!\nFull Time Position: Monday - Friday 8:30am-5pm\nSalary starts at $20/hour\nReports directly to the Social Services Manager\nAs a Social Services Coordinator at The Springs Health Center, you'll play an essential role in supporting residents and their families through life's transitions, helping ensure every resident receives the emotional, social, and psychological support they need to thrive.\nIf you are compassionate, organized, relationship-focused, and passionate about advocating for seniors, we'd love to meet you.\nAs our Social Services Coordinator, you will:\nBuild meaningful relationships with residents and their families\nAssess residents' social, emotional, psychological, and adjustment needs\nDevelop and coordinate individualized care plans\nProvide support and guidance during challenging life transitions\nAssist residents and families with discharge planning and community resources\nSupport residents experiencing dementia, behavioral health concerns, grief, or loss\nCollaborate with nursing, therapy, physicians, and interdisciplinary team members to ensure quality resident outcomes\nMaintain accurate documentation and ensure compliance with federal, state, and community regulations\nAdvocate for residents while promoting dignity, independence, and quality of life\nHere are a few of the qualifications we need you to have:\nBachelor's Degree in Social Services or related field required;
