Leveraging our extensive global network, we connect clients with highly qualified professionals, offering tailored services to meet our clients' unique business needs.\nThe Opportunity:\nWe are seeking a highly skilled Registered Nurse Case Manager in Indiana to administer skilled nursing care to patients on an intermittent basis in their place of residence.\nKey Responsibilities\nProvides services in accordance with the plan of care.\nMakes the initial evaluation visit and regularly reevaluates the patient’s nursing needs.\nInitiates the plan of care and necessary revisions.\nProvides those services requiring substantial specialized nursing skills.\nInitiates appropriate preventive and rehabilitative nursing procedures.\nPrepares clinical and progress notes for each patient visit and summaries of care conferences on his/her patients in a timely manner as per Agency policy.\nCoordinates services.\nInforms personnel of changes in the condition and needs of the patient.\nCounsels the patient and family/significant others in meeting nursing and related needs.\nProcesses orders and notifies physician of patient needs and changes in condition. Completes certification/recertification orders and discharge summaries.\nDetermines the amount and type of nursing needed by each individual patient. \nSupervises and teaches other nursing personnel. \nConducts patient care conferences on patients assigned to his/her care. \nParticipates in peer review and Quality Assessment and Performance Improvement as assigned. \nGives total patient care as needed; Takes on-call duty nights, weekends and holidays, as assigned.\nCompletes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition in accordance with Agency defined time frames. \nAppropriately utilizes ICD-10 codes. \nPerforms other related duties as assigned.\nRequired Skills/Abilities:\n Ability to exercise initiative and independent judgment. \nAbility to work independently and leverage community resources to benefit clients. \nExcellent interpersonal, written and verbal communication skills. \nAbility to thrive in a fast-paced and ever-changing environment. \nEnergetic team player with a positive attitude and curious mindset.\nEducation & Experience:\nGraduate of an accredited nursing school.
