These recognitions highlight LVHN's commitment to teamwork, compassion, and technology with an unrelenting focus on delivering the best health care possible every day.\nWhether you're considering your next career move or your first, you should consider Lehigh Valley Health Network.\nSummary\nCoordinates the services for solid organ transplant patients and communicates with the multidisciplinary patient care team, nephrologists, and dialysis centers, staff, and other healthcare providers.\nJob Duties\nCoordinates admission of potential donor and recipient to hospital for surgery.\nCoordinates care for transplant patients in the inpatient setting.\nCoordinates organ transplant with Organ Procurement Organization, transplant candidate, physicians, and hospital staff.\nEvaluates potential transplant candidates and coordinates pre-transplant evaluation for potential transplant recipients.\nManages pre-transplant patient care, evaluation, and management in the outpatient setting.\nParticipates and coordinates in post transplant care of patients.\nParticipates in 24 hour on call availability for transplant program coordinators and physicians.\nReviews medical history and suitability to donor; coordinates schedules of medical tests and reviews test results to ensure optimal outcome for patient.\nObtains and reviews referral information from nephrologists, dialysis units, or other referral sources.\nEnsures admission and pre-operative studies are completed and results provided to physicians.\nFacilitates clinical protocols for post transplant medication (immunosuppression, etc) management.\nMinimum Qualifications\nBachelor’s Degree in Nursing \n4 years Experience as a nurse in the medical, surgical, and or dialysis field. \nDemonstrated knowledge of current transplant-related medical information. \nRN - Licensed Registered Nurse_PA - State of Pennsylvania Upon Hire \nPreferred Qualifications\nMaster’s Degree in Nursing \nPhysical Demands\nLift and carry 40-50 lbs. Examples: Push/pull patients on bed, stretcher (requires 29 lbs. push force), lateral transfers up to 50 lbs. of the patient's weight. Frequent to continuous standing/walking.
