Together, our vision is to improve the health of people in our community by creating an interconnected system of care across the continuum, from prenatal care to birth to end of life and bereavement. The System Planner will report to the Program Director of Community Integration and be accountable to the WYR OHT Collaboration Council. As a key resource to the OHT, the System Planner will build and maintain strong relationships with internal and external partners and will employ leadership and best practices to plan and implement the OHT. In this role, you will be an innovative leader who excels in \"engaging people fully\", with planning and decision-making that enables a culture of collaboration amongst partners.
