We are committed to Diversity, Equity, and Inclusion, providing organizational-wide social opportunities, and constantly improving our ongoing efforts to positively impact our members' lives.\nWhat You'll Do:\nProvide administrative support to the members of the CM team.\nCollect data for Health Risk Assessments (HRA).\nScreen for eligibility and benefits.\nIdentify members without a PCP and refer to Member Services.\nScreen members by priority for CM services and refer as appropriate.\nPerform transition of care duties to include, but not limited to, contacting the members’ PCP, Medical POA, or other medical providers for information pertaining to special needs.\nDocument all encounters and contacts made on behalf of clients; complete and submit monthly reports; maintain comprehensive electronic member records.\nMotivate members to be active and engaged participants in their health and overall well-being.\nProvide educational promotion, member follow-up, arrange PCP visits, and perform care coordination under the direction of the Care Manager.\nAssist members in assessing health-related services, including but not limited to obtaining a medical home, providing instruction on appropriate use of the medical home, overcoming barriers to obtaining needed medical care, and/or social services.\nPerform tasks as directed by the CM team to promote member compliance, such as verifying appointments or obtaining medical records.\nFacilitates member access to community resources, including, but not limited to, finding housing, food, clothing, prenatal classes, parenting, and providers to each life skill, and relevant mental health services.\nWork collaboratively and effectively within a team. Establishes positive, supportive relationships with participants and provides feedback to other members of the team. Builds and maintains positive working relationships with members, providers, nurse care managers, social workers, agency representatives, and office staff, from diverse cultural and socio-economic backgrounds.\nRequirements\nStrong interpersonal, communication, and customer service skills and the ability to work effectively with a wide range of constituencies in a diverse community.\nKnowledge of community agencies and resources.\nKnowledge of medical terminology.\nWorking knowledge of multi-system outreach programs related to health care delivery, clinical education, and health-related services.\nKnowledge of transportation and other barriers to care that members may face.\nRequirements\nEducation and Work Experience:\nHigh school diploma or equivalent\n2+ years serving the Medicaid population, preferably in the MD/DC area.\n2+ years OB/GYN experience\nKnowledge of Medical Terminology\nEEOC Statement:\nFollowing applicable federal, state, and local laws, MCMI prohibits discrimination in employment based on race, color, religion, sex (including pregnancy, gender identity, and sexual orientation), national origin, age, disability, genetic information, or any other characteristic protected by law.
