We are unable to support US visa sponsorship at this time.\nBackend Engineer (PHP / Laravel)\nThe Backend Engineer (PHP / Laravel) designs, develops, and maintains scalable and reliable server-side applications to ensure the highest standard of system performance and stability. As a Backend Engineer, you will take ownership of features and services, troubleshoot complex issues, optimize existing code, and contribute to architectural decisions. You will work closely with cross-functional teams, including front-end dev, product, QA, and DevOps, collaborating both synchronously and asynchronously to deliver high-quality software, efficiently.\nWe are looking for a skilled, detail-oriented engineer who is comfortable with full-stack collaboration, enjoys solving challenging problems, and thrives in a remote-first, fast-paced environment.\nKey Responsibilities\nDesign, build, and optimize high-throughput RESTful APIs in PHP using the Laravel framework, writing efficient PostgreSQL queries and schema designs to support data-intensive clinical workflows. Contribute to new feature development and platform improvements within an Agile/Scrum process.\nBuild and maintain integrations with third-party APIs and internal services, deploying and running services in Docker containers across AWS (or comparable cloud) environments to ensure secure, reliable data exchange.\nWrite automated tests (unit, integration, and API-level) for PHP/Laravel back-end functionality and critical clinical workflows, and integrate them into CI/CD pipelines using Git/GitHub-based workflows and Docker to enable continuous testing and faster, more reliable releases.\nWrite and maintain clear technical documentation for troubleshooting guides, fixes, API integrations, and support processes, version-controlled through Git/GitHub for easy team access and updates.\nMonitor application health, performance, and error logs using tools such as Rollbar and AWS CloudWatch to proactively identify and address potential issues before they affect patients or providers.\nIdentify, document, and track software defects, analyzing logs and debugging to pinpoint root causes, and work with the development team within Agile/Scrum sprints to prioritize and ensure timely resolution.\nActively participate in code reviews to uphold coding standards and engineering best practices. Continuously evaluate and improve our development processes, CI/CD pipelines, and tooling (Docker, monitoring/logging systems, cloud infrastructure) to enhance team efficiency, code quality, and release velocity.\nIdentify opportunities to formalize and document internal processes, workflows, and troubleshooting playbooks, building a knowledge base that supports faster onboarding and more consistent support.\nYour Qualifications\n5+ years of professional experience with PHP and Laravel framework.\nExperience working with AWS services like Lambdas, SES, Workers, etc.\nSolid knowledge of RESTful APIs, with ability to write automated API tests.\nDegree in Computer Science, Engineering, or equivalent combination of education and experience.\nDeep experience with SQL databases (preferably PostgreSQL), including schema design, queries, and performance optimization.\nAbility to use AI tools to boost productivity but does not rely on it to be able to perform.\nExperience working with version control systems (Git) and collaborative workflows (GitHub), including CI/CD pipelines and ability to integrate automated tests into the deployment process.\nExperience with Docker\nKnowledge of monitoring tools (e.g., Rollbar, CloudWatch, etc.)\nLocated in the US and able to work full-time for a US employer without requiring visa sponsorship.
