 Job Summary

Infinite Medical P.C. is seeking an experienced Nephrology Nurse Practitioner to provide comprehensive kidney care to residents in skilled nursing and long-term care facilities, as well as patients receiving dialysis treatment. This part-time role is ideal for an advanced practice provider with nephrology or dialysis experience who is passionate about delivering high-quality, patient-centered care in a collaborative healthcare environment.

Responsibilities

  • Provide nephrology-focused assessments and ongoing management for patients with acute and chronic kidney disease.
  • Evaluate patients, develop treatment plans, and manage nephrology-related conditions in collaboration with the supervising or collaborating nephrologist, as applicable.
  • Monitor dialysis treatment plans, adequacy, tolerance, and patient outcomes.
  • Perform comprehensive patient assessments, review medical histories, and conduct physical examinations.
  • Interpret laboratory results, diagnostic studies, and clinical findings to support individualized treatment plans.
  • Manage associated conditions such as hypertension, anemia, fluid imbalance, and electrolyte disorders.
  • Adjust medications and treatment plans within scope of practice and in collaboration with the nephrology team.
  • Monitor patients during and after dialysis treatment and respond appropriately to changes in clinical condition.
  • Collaborate with physicians, nursing staff, dialysis technicians, facility staff, and other healthcare professionals to ensure coordinated patient care.
  • Participate in interdisciplinary care meetings and quality improvement initiatives.
  • Educate patients and families regarding kidney disease, dialysis treatment, medication adherence, diet, fluid restrictions, and lifestyle modifications.
  • Serve as a clinical resource and point of contact for patients, families, and facility staff regarding nephrology-related concerns.
  • Maintain accurate, timely, and compliant clinical documentation.
  • Coordinate care with specialists, dialysis centers, primary care providers, and facility teams.
  • Support patient advocacy and continuity of care across settings.

Requirements

  • Master’s or Doctoral degree in Nursing from an accredited Nurse Practitioner program.
  • Current and unrestricted Nurse Practitioner license in the state of Texas.
  • Current national board certification as a Nurse Practitioner.
  • Current DEA registration, if required for the position.
  • Nephrology, dialysis, renal, or related clinical experience strongly preferred.
  • Experience caring for adult and geriatric populations preferred.
  • Skilled nursing, long-term care, or post-acute care experience preferred.
  • Strong clinical assessment, communication, and decision-making skills.
  • Ability to work independently while collaborating effectively with physicians and interdisciplinary teams.
  • Commitment to patient-centered care and improving outcomes for patients with chronic kidney disease.
  • Prolonged standing and walking may be required.
