In this role, you will provide comprehensive rehabilitation care for residents in skilled nursing and long-term care facilities, focusing on pain management, functional improvement, and quality of life. You will collaborate closely with primary care providers, therapists, and interdisciplinary teams to develop individualized care plans and support optimal patient outcomes.\nResponsibilities\nConduct comprehensive evaluations of patients with functional impairments, disabilities, and chronic conditions\nDiagnose and manage rehabilitation and pain-related conditions\nDevelop and implement individualized rehabilitation care plans in collaboration with interdisciplinary teams\nPrescribe and oversee physical, occupational, and other therapeutic interventions\nPerform procedures within scope of practice (e.g., joint injections, pain management techniques) as applicable\nCommunicate treatment plans, progress, and recommendations to patients, families, and facility staff\nMonitor patient progress and adjust care plans to optimize outcomes\nEnsure timely follow-up on treatment plans and interventions\nDocument clinical findings, care plans, and outcomes accurately in the medical record\nMaintain compliance with HIPAA, infection control standards, and facility policies\nCollaborate with physicians, advanced practice providers, therapists, and nursing staff to support coordinated care\nParticipate in quality improvement initiatives and support best practices in rehabilitation care\nTravel to partner skilled nursing and long-term care facilities as assigned\nRequirements\nDoctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited medical school\nCompletion of an ACGME-accredited residency in Physical Medicine & Rehabilitation (PM&R)\nBoard-certified or board-eligible in Physical Medicine & Rehabilitation\nActive and unrestricted Physician license in the state of practice\nCurrent ACLS and BLS certifications preferred\nPrior physician experience in rehabilitation medicine, pain management, or related settings preferred\nExperience working in skilled nursing or long-term care settings is a plus\nStrong clinical assessment and functional evaluation skills\nExcellent communication and interdisciplinary collaboration abilities\nExperience providing care to the geriatric population\nAbility to travel to multiple facilities as needed\nProlonged standing/walking. Assisting with patient positioning. Occasional lifting.
