Together, we champion continuous innovation and collaboration in our shared mission to redefine senior care across the country.\nJob Summary\nInfinite Medical Group is currently hiring a Primary Care Nurse Practitioner/Physician’s Assistant (NP/PA) to provide on-site care to residents in long-term care settings. This is a full-time position ideal for a provider who values meaningful patient relationships and a collaborative care environment.\nResponsibilities\nProvide primary care services to residents in skilled nursing and long-term care facilities\nPerform physical exams and medical history assessments\nDiagnose and treat chronic and acute health conditions\nOrder and interpret diagnostic tests and lab results\nDevelop and adjust individualized care plans\nPrescribe medications and monitor treatment effectiveness\nCollaborate with facility staff and interdisciplinary teams\nEducate patients and families about treatment options and care goals\nEnsure a safe, respectful, and supportive environment for residents\nRequirements\nActive and unrestricted Nurse Practitioner, Physician Assistant, or Physician license\nState-specific license in good standing in Vermont\nMinimum 6 months of experience in long-term care or a skilled nursing facility\nStrong communication, clinical, and critical thinking skills\nPassion for elder care and person-centered healthcare\nProlonged standing/walking. Occasional lifting up to 20 lbs. Computer work. Lifting/moving patients.
