We have a full-time opportunity available to work within the SA Community Care (SACC) program.\nIn this virtual role, you’ll sit at the centre of the care journey, connecting patients, families, and providers to ensure care is coordinated, timely, and genuinely patient-centred.\nKey Responsibilities\nConducting clients initial phone assessment, care planning and goal setting\nCoordination of in-home services to support clients receiving care at home\nManage escalations and provide support for our frontline nurses in the community\nManaging consumer care throughout the consumer journey (on-boarding, triaging referrals, initial care planning within scope of practice, care plan review and discharge)\nProvision of telephone support, education and advice\nDischarge planning including referrals to long term services\nMonitor patient progress, identify risks, and escalate concerns appropriately\nCollaborate with multidisciplinary teams to ensure continuity of care\nMaintain accurate clinical documentation and reporting in line with standards\nYou will be rostered across 7-days working between 7am and 9pm - the shifts are 7am to 3pm, 10am to 6pm, 12pm to 8pm and 1pm to 9pm. This role is based in Adelaide but will consider applicants interstate.\nAbout you\nRegistered Nurse with current unrestricted AHPRA registration\nExperience working in an acute setting (community/hospital)\nStrong understanding of care coordination, case management, or complex care (highly desirable)\nDigital proficiency and ability to work virtually\nExcellent communication and relationship-building skills\nConfident working independently while collaborating within a multidisciplinary team\nHighly organised with strong clinical judgment and problem-solving skill\nWhat’s next?\nIf you have the skills and experience we are looking for we will be in contact.
