Our Care Coordinators assist Medicaid-eligible individuals in coordinating both behavioral and/or physical health care, regardless of diagnosis. They work with those who qualify to ensure they access the best possible services for physical and mental health. These services are provided through our partnership with Hudson River Health Care, our Health Home provider. Care Coordinators collaborate with other providers to develop personalized wellness plans, focusing on rehabilitation, recovery, and linking individuals to essential clinical services for improved daily health management.\nPosition Type: Full Time-40 Hours/Non-Exempt\nPay Rate: $22.00 per hour with an increase to $24.00 per hour after one year of employment\nWork Location: Greene County, NY\nShift Schedule: Monday-Friday 8:30am-4:30pm\nJob duties include:\nCoordinating with service providers and health plans to secure necessary care for members.\nThe Care Coordinator role will involve 75% fieldwork, which includes, but is not limited to, traveling to clients’ homes and within the community to conduct assessments and manage ongoing care. \nProvide transportation to clients when necessary.\nConnect and refer clients to appropriate services that support their care plan and treatment goals, including medical/behavioral health care, patient education, and self-help recovery and self-management. \nConduct case reviews with interdisciplinary team to monitor/evaluate client status/needs. \nAdvocate for services and assist with scheduling of needed services. \nCollaborate with treating clinicians to ensure services are delivered and that changes in medical treatment or conditions are addressed. \nMonitor and support clients in making scheduled medical/behavioral/social service appointments. \nProvide crisis intervention when needed. \nRevise care plans and goals as necessary. \n**This position offers some flexibility to work remotely based on program needs and supervisor approval**\nRequirements\nMust have a bachelor's degree in a social service or counseling discipline such as social work, psychology, nursing, rehabilitation, community mental health social work, psychology, or related field OR Registered Nurse with bachelor’s degree and at least 2 years’ experience in providing direct services in the mental health field or in linking clients to a broad range of services in the community.\nExperience in providing direct service in the mental health or social service field and linking clients to services in the community is necessary. \nCandidates must have reliable transportation to travel to client locations and other necessary work sites.\nProficiency in, or ability to learn, online technology systems is critical. \nAbility to work with a diverse population of urban, suburban and rural cultures and with community members from varying socioeconomic backgrounds. \nDemonstrate a commitment to Diversity, Equity, and Inclusion through continuous development, modeling inclusive behaviors, and proactively managing bias. \nMust be able to work regularly outside the office and in the field to engage with clients. \nExcellent oral and written communication skills. \nFamiliarity with Greene County and knowledge of managed care is a plus. \nRegular and steady attendance is a requirement for all positions.\nCandidates from historically underrepresented groups are encouraged to apply.\nAll offers are contingent upon successful Statewide Central Registry (SCR), fingerprint clearance and possession of a valid NYS driver’s license with a MVR eligible for agency insurance coverage.\n As an Equal Opportunity Employer, we do not discriminate in our employment practices due to an applicant's sex, race, color, religion, national origin, citizenship, age, creed, disability, veteran status, marital status, familial status, military status, domestic violence victim status, sexual orientation or gender identity/expression (including transgender status), predisposing genetic characteristics/genetic information or carrier status, ancestry, ethnic group identification, ethnic background, traits historically associated with race, reproductive health decision making, or any other category protected by applicable federal, state, or local law.
