Together we’re building hope and possibility for all.\nFind out more about us and our purpose.\nYour Opportunity\nEvery person deserves hope, connection, and support during life's most difficult moments.\nThe Way Back Program is a non-clinical support service focused on providing rapid, practical psychosocial support to people experiencing a suicidal crisis or who have attempted suicide.\nWe are seeking a compassionate and dedicated Care Coordinator to join our team. In this rewarding role, you will work alongside Peer Workers and community partners to provide person-centred support that empowers individuals to achieve their goals and reconnect with their communities. As a Care Coordinator, you will work closely with people experiencing suicidal distress and their support networks, helping them access the services, resources and connections they need to support their recovery and healing. You will provide holistic, strengths-based support that promotes independence, resilience, community participation and wellbeing\nYour key responsibilities will be to:\nUndertake initial intake and safety planning for clients and work with individuals and their families to create individualized case plans.\nProvide ongoing support sessions with clients and review progression and safety as required.\nSupport and promote family, carers, friends and kin involvement in safety and support planning in collaboration with clients\nLiaise with other support services and organisations to coordinate the requirements of the case plan.\nDeliver group programs which support the practical needs of the clients\nAssist clients in the process of transition out of the service into independence or other services.\nEnsure documentation is completed and updated in line with Mission Australia protocols.\nRequirements for Success\nRelevant tertiary qualifications and/or significant relevant work experience\nExperience working with people experiencing mental health challenges, suicidality, trauma and psychosocial support needs\nDemonstrated experience supporting individuals from diverse backgrounds, including those experiencing disadvantage and social exclusion\nKnowledge of local crisis intervention and community support services and the ability to connect people with the right supports\nExperience working with a peer workforce, with a focus on trauma-informed, recovery-oriented practice and valuing lived experience\nExcellent communication, relationship-building and case management skills.\nComputer literacy preferably using Windows.\nCurrent NSW Driver’s License\nCurrent First Aid Certificate\nBefore starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.\nYour benefits\nTo ensure our employees feel valued, empowered, and celebrated, we provide a range of employee benefits including:\nRewarding work - every day you know you’re making a real difference\nNot-for-profit salary packaging, reducing your tax (details via AccessPay)\nFree external, confidential counselling and access to chaplaincy support\nVolunteering opportunities \nOpportunity to purchase additional annual leave\nExtra leave including Mission Australia Thank You Day & Work Anniversary Day off\nOur culture\nOur innovative and inclusive culture will ensure you feel valued at Mission Australia as you help Australians in need and develop in your career.\nWe prioritise the safety and wellbeing of everyone, both within our team and the individuals we serve.\nWe are committed to creating a diverse and inclusive working environment.
