Duties include:\nIntegrating, overseeing and monitoring all aspects of the physical and behavioral health needs for adults experiencing severe mental illness.\nCompletion of individual assessments and development of a person-centered plan of care.\nReferral coordination and linkage to community services.\nCollaboration with other community service agencies and health care providers.\nIn person home and community based engaged with transportation to members using the agency’s vehicles.\nPromote a culture of inclusion and belonging.\nRequirements\nRequirements Skills/Qualifications for the Care Coordinator include:\nBA/BS in human services required.\nMinimum of 2 years human services with individuals experiencing mental illness, substance use disorder or developmental disabilities;\n1 year providing in person case management preferred (defined as experience in assessment, care planning, knowledge of benefit entitlements, linkage to community providers and services).\nA demonstrated ability to function as a member of an interdisciplinary team.\nValid and insurable driver's license. Agency vehicles are available for transporting clients. Must have a personal - vehicle to use for business purposes when not transporting clients.\nBenefits\nBenefits Salary and Schedule of the Care Coordinator:\n$50,876.80/year, hourly position.\n40 hours per week. Monday-Friday.
