Write an event note that reflects participant activities, goals, interventions, and outcomes.\nDevelop service plans and/or person-centered plans as appropriate, and screens referrals in coordination with the other Assertive Community Treatment Team members.\nMeet regularly to review the total caseload, problem-solve, develop plans, and determine daily duty assignments.\nAttend and actively participate in meetings and training as required. Maintain certification in all agency, state and federal training requirements.\nDemonstrate knowledge of and comply with all agency policies and procedures, as well as state and federal statutes and regulations related to specific program areas.\nFollow service definition guidelines for services being provided.\nComplete all other relevant responsibilities as assigned by the supervisor.\nDriving and travel may be required.\nBenefits:\nWe offer a competitive and flexible total rewards package designed to support the diverse needs of our team members.
