This highly collaborative position combines patient engagement, care coordination, and population health management to help patients navigate complex healthcare needs, improve access to care, and achieve better health outcomes.\nThe CMA serves as a trusted resource for patients, caregivers, providers, and interdisciplinary care team members. Through proactive outreach and coordination, the CMA helps ensure that patients receive timely care, remain engaged in treatment plans, and have access to the resources needed to successfully manage their kidney health.\nThis is an excellent opportunity for an LPN who enjoys building relationships with patients, coordinating care across multiple settings, and making a direct impact on quality outcomes and patient experience.\nWork Location:\nThis position is primarily remote and focuses on telephonic outreach and care coordination with CKCC program participants. Occasional travel to provider offices, healthcare facilities, team meetings, or other sites of care may be required based on program or business needs.\nKey LPN role in CKCC program includes:\nEngage and educate patients about the CKCC program, ensuring understanding of program benefits and completing enrollment and consent activities.\n\nBuild meaningful relationships with patients and caregivers through ongoing outreach and support.\n\nCoordinate care across providers, dialysis centers, hospitals, and community resources.\n\nAssist patients in overcoming barriers to care by facilitating:\nAppointment scheduling and follow-up visits\n\nMedication access and prescription refill coordination\n\nTransportation arrangements\n\nReferrals to appropriate healthcare and community services\n\nSupport smooth transitions of care following hospitalizations through post-discharge outreach and scheduling of timely follow-up appointments.\n\nFacilitate completion of Patient Activation Measure (PAM) assessments to support patient engagement and self-management.\n\nCoordinate referrals and transitions for patients initiating outpatient dialysis services.\n\nGather and organize clinical and non-clinical information from patients, caregivers, providers, electronic health records, and other systems to support care management activities.\n\nCollaborate closely with multidisciplinary care teams to identify opportunities for intervention and improve patient outcomes.\n\nServe as a resource for patients, physicians, caregivers, and internal stakeholders regarding CKCC program services and processes.\n\nRequirements:\nAssociates degree required. Bachelor’s Degree preferred.\n\nLPN Licensure Required.\n\nCurrent active license/registration in New York State.\n\nMedical Management experience in a managed health care setting preferred.\n\nAppropriate training and experience in case management preferred.\n\nMontefiore Medical Center is an equal employment opportunity employer.
