In collaboration with physicians and the multidisciplinary healthcare team, the LVAD Coordinator will be responsible for the planning, evaluation, coordination, implementation, and facilitation of all aspects of care for patients and families requiring mechanical circulatory support and heart transplantation.\nResponsibilities Include:\nCoordinates and provides education to ventricular assist device (VAD) patients and their families.\nCoordinates the comprehensive inpatient and outpatient clinical management of the ventricular assist device population.\nEnsures adherence to established protocols and program certification requirements.\nParticipates in remote on-call coverage for approximately four shifts per month, providing emergency transplant/LVAD hotline management.\nSchedule:\nFour-day workweek.\nWeekends and holidays off, unless scheduled for remote on-call coverage.\nRegular schedule: 8:30 AM-6:30 PM.\nWednesday schedule: 7:30 AM-5:30 PM.\n37.5 hours per week.\nEducation and Skills Required:\nMaster's degree in Nursing required.\nCurrent New York State license and registration as a Registered Professional Nurse (RN).\nCurrent New York State Nurse Practitioner specialty certification and registration OR current certification from the National Commission on Certification of Physician Assistants (NCCPA).\nThree (3) years of clinical experience in a highly specialized, complex care program preferred.\nClinical experience in a highly specialized cardiac care program preferred.\nMontefiore Medical Center is an equal employment opportunity employer.
