Our mission is to promote independence and quality of life for those we serve.\nJob Summary:\nWe are seeking a dedicated and compassionate Case Coordinator to join our Clinical Services team. The Case Coordinator serves as the central link between the clinical, residential, educational, and external teams to ensure consistent and effective service delivery. This role combines elements of case management and QDDP to promote individualized, person-centered treatment for each student in alignment with Indiana DCS standards.\nThe Case Coordinator supports treatment planning, monitors implementation, facilitates communication among all stakeholders, and actively advocates for every student’s best interests. This position also plays a key role in discharge and aftercare planning to ensure continuity of care and positive long-term outcomes.\nKey Responsibilities:\nCare Coordination & Case Management\nDevelop, implement, and monitor Individualized Treatment Plans and Behavioral Support Plans in collaboration with internal and external team members.\nEnsure services are person-centered, clinically appropriate, and compliant with Indiana DCS standards and agency philosophy.\nMonitor and document progress toward treatment goals; revise plans as needed.\nCoordinate student admissions, transitions, and discharge planning to support permanency and continuity of care.\nProvide Aftercare Services for DCS students upon discharge, when applicable.\nMeet individually with assigned students at least monthly to support engagement and treatment coordination.\nParticipate on the floor with students daily to support programming and build rapport.\nCommunication & Stakeholder Engagement\nServe as the primary contact for students on assigned caseloads, maintaining communication with guardians, DCS case managers, court representatives, and other partners.\nProvide ongoing updates, required notifications, and incident reporting per DCS policy.\nFacilitate and attend meetings including CST, TTR, IEP, transition planning, treatment reviews, and court hearings (virtual or in person).\nSupport and supervise family or external team communication when needed.\nPromote positive, professional relationships that support student success and family engagement.\nClinical Collaboration & Oversight\nPartner with program and direct care staff to ensure consistent implementation of Behavioral Support Plans.\nParticipate in assessments such as Functional Life Skills and Ansell-Casey to identify strengths and needs.\nSupport the development of therapeutic programming appropriate to each student’s age, functioning level, and cultural background.\nAssist with crisis response and follow-up support to ensure learning and team alignment.\nReporting & Documentation\nMaintain accurate and timely documentation in accordance with Indiana DCS, COA standards, licensing requirements, and agency policy.\nPrepare treatment updates, transition plans, progress summaries, and placement reports.\nEnsure confidentiality and secure record-keeping standards are followed at all times.\nProfessional Conduct & Team Support\nProvide guidance and support to staff regarding treatment goals and case coordination processes.\nParticipate in training and orientation activities as assigned.\nDemonstrate professionalism, ethics, and positive teamwork in all interactions.\nAccept supervision and engage in regular consultation with leadership.\nComprehensive Employee Benefits Package\nAt T.C.
