North Bay Regional Center is a nonprofit private corporation that contracts with the Department of Developmental Services to coordinate services and supports for individuals with developmental disabilities. At North Bay, we’re promoting opportunities by supporting choices for people with developmental disabilities.\nNorth Bay Regional Center serves over 13,000 people living within the geographic boundaries of Napa, Sonoma and Solano counties. Guided by our NBRC Mission and Values to assist people with developmental disabilities to obtain the services and supports needed to live as others do in the community.\nPosition Description:\nIn the role of Social Services Case Manager (Service Coordinator) you will be responsible for managing the delivery of services to regional center clients including those with intensive physical or emotional needs, ensuring that the client’s Individual Program Plan and its objectives are met.\nJob Responsibilities\nOur Social Services Case Managers/Service Coordinators are responsible for using person-centered planning to assists clients in use of public and private resources before using regional center funds to purchase services.\nAdditional responsibilities of the Social Services Case Manager include:\nMonitoring vendor services and verifying that services are delivered in accordance with the purchase of service authorizations\nEvaluating the effectiveness of clients’ Individual Program Plans; ensuring that appropriate community resources are made available to the client\nConducting annual reassessments of clients’ developmental status in conjunction with the interdisciplinary team\nConducting client assessments on an occasional basis\nRequirements\n MINIMUM EDUCATION AND EXPERIENCE\nTo qualify for the Social Services Case Manager role you must enjoy working with clients and assisting people with developmental disabilities. Additional requirements of the Social Services Case Manager include:\nBachelor’s Degree in a field which offers content related to developmental disabilities (i.e. Social Work, Psychology, Counseling, Special Education, and/or Nursing). Minimum education requirement may not be waived.\nMaster’s Degree preferred\n3 years’ experience working with individuals with developmental disability\nBilingual language ability (Spanish to English) is preferred.\nA note regarding NBRC’s expectations regarding remote, hybrid, and home-based work…\nAll employment at NBRC requires face-to-face, in-person interactions with NBRC clients, family members, community, and co-workers. Candidates for employment must live within commute distance to their preferred NBRC office, as well as the geographical location of their assigned clients.
