eligible employer \nEligible clinical positions may qualify for the National Health Service Corps (NHSC) Loan Repayment Program\nEssential Functions:\nPerforms all essential functions of the Community Care Manager position, including care management, care coordination, direct service delivery, and facilitation of therapeutic and psychoeducational groups as assigned.\nProvides individual interventions to develop or enhance clients' ability to make informed, independent choices, improve functioning, and achieve person-centered goals.\nCoordinates services across behavioral health, physical health, community providers, and natural supports to promote integrated, person-centered care and positive client outcomes.\nParticipates as a member of the multidisciplinary treatment team in the development and implementation of person-centered care plans, safety plans, and relapse prevention plans as indicated.\nDemonstrates the ability to recognize and assess suicide risk, behavioral health symptoms, substance use concerns, physical health needs, and other crisis situations, responding appropriately within agency standards.\nProvides comprehensive outreach, engagement, care coordination, and follow-up services for assigned individuals.\nProvides in on-call and crisis response/support responsibility as assigned. \nDemonstrates the ability to engage and develop rapport with individuals presenting with a variety of behavioral health, substance use, and medical needs.\nProvides services that are person-centered, culturally responsive, non-judgmental, and welcoming to diverse populations.\nServes as a team lead by providing day-to-day guidance, mentoring, support, and clinical consultation to Care Management staff.\nProvides consultation and support to Care Management staff regarding care coordination, service planning, community resources, documentation, engagement strategies, and complex client needs.\nAssists leadership in coordinating daily program operations to promote quality service delivery, efficient workflow, appropriate staffing coverage, and program growth.\nSupports program compliance with agency policies, contractual requirements, accreditation standards, and state and federal regulations.\nAssists with program monitoring activities including documentation review, quality improvement initiatives, audits, productivity tracking, referral management, census management, and data collection.\nAssists with onboarding, orientation, training, coaching, and ongoing professional development of Care Management staff.\nSupports specialized programs, grants, contracts, and agency initiatives within Care Management as assigned.\nCollaborates with multidisciplinary teams, internal departments, community partners, referral sources, managed care organizations, and other stakeholders to promote effective care coordination and positive program outcomes.\nDemonstrates excellent organizational, communication, leadership, problem-solving, and time management skills while balancing direct client care with administrative responsibilities.\nDocuments all services within expected timeframes in accordance with service definitions, regulatory standards, quality improvement goals, and agency expectations.\nConsistently meets established productivity, client care, documentation, and program performance expectations.\nParticipates in and completes all required training, supervision, and professional development activities in an accurate and timely manner.\nOther: Completes other administrative support duties, leadership responsibilities, and special projects as developed and assigned.\nQualifications\nEducation and Experience:\nMaster’s Degree in a human services related field OR \nBachelor’s degree in a human services related field and 1 year relevant experience working with the target population preferred OR \nHigh School Diploma or GED with a minimum of 3 years of experience working with the target population OR \nCertified Peer Specialist (CPS) or Certified Family Specialist (CFS)\nEXPERIENCE:\nDependent on type of education (see above). \nA minimum of two years working with target population. \nOne year demonstrated supervisory experience preferred.\nOTHER: Valid drivers’ license and meets the driving privileges guidelines established in HR Policy 12.6 “Vehicle Safety and Client Transportation”\nKnowledge and Skills:\nDemonstrated success in the following areas:\nAbility to demonstrate all functions of clinical program staff. \nAbility to manage time to meet all deadlines.\nAbility to recognize and identify symptoms of mental illness and suicide and risk potential and respond appropriately. \nGood verbal and written skills.\nBilingual (English/Spanish) is preferred but not required.\nValid driver’s license-Yes\nCPR/First Aid-Yes\nCPI -
