Your evaluations will drive the development of personalized care plans, ensuring the highest standard of care and well-being of our residents. This role will cover Metro Albany area Hospitals and for placement with The Pines at Glens Falls and Catskill Skilled Nursing Facilities.\nKey Responsibilities:\nPerform comprehensive assessments of physical, mental, and emotional health to determine the appropriate care setting\nEvaluate functional abilities and care needs through detailed observations and assessments\nCollaborate with interdisciplinary teams to develop individualized care plans\nDevelop and maintain positive relationships with area hospitals and community agencies\nEstablish and maintain relationships with social workers, case managers, and other contacts in the healthcare and payor community\nEducate residents and families about facility services, procedures, and care expectations\nMonitor and reassess residents' progress and update care plans accordingly\nAdvocate for residents' well-being and coordinate with healthcare providers to address needs\nDocument assessments and care plans accurately and in a timely manner\nIf you are a skilled and empathetic healthcare professional with a passion for assessing and advocating for patient care needs, we invite you to join our team as a Clinical Evaluator.
