In addition, we provide advice and guidance to Social Workers, practitioners, and a wide range of professionals regarding Short Breaks, eligibility, and commissioned services.\nAs a Disability Services Access Coordinator, you will play a key role in ensuring that families receive a fair, transparent and needs led Short Breaks offer. You will review assessments, identify support needs, and work directly with families to ensure service providers are meeting expectations. You will also act as the liaison officer for a number of commissioned Short Breaks providers, supporting contract monitoring, performance reporting and service development.\nAbout the Role\nYou will be the first point of contact for families and professionals requiring more detailed information about Short Breaks, assessments, resource allocation, or issues with service provision.
