Scope of Position

TheMedication Management /Transitions of Care(TOC)Pharmacy Technicianprovides a valuable patient-centered service by focusingonfacilitatingTOC,includingobtainingand documentingacomplete andaccurateadmissionmedicationhistoryand allergy informationin the electronic medical record (EMR)as well asassisting with the discharge medication process.TheTOC Pharmacy Techniciancompletesthe admission medicationhistoryduring aninpatient admission,ambulatory encounter, pre-admission testing, surgeryencounter, orwithin a procedural areaunder the supervision of a pharmacist, physician, oradvancepractice provider.Informationcan be gathered froma face-to-faceor telehealthinterview withthe patient chart andcommunication with patients, caregivers, physicians,retailpharmacies, andlong-termcare facilities.The TOC Pharmacy Technicianwill make agood faitheffort to ensure the documented information is complete and accurateby identifying and facilitatingcorrection of medication discrepanciesor escalating to a pharmacistor providerfor follow up, if needed,thereby aiding in the hospital’s medication reconciliation efforts.Throughout the hospital admission, the TOC Pharmacy Technician is responsible forcollaborating withthe patientandcare teamtofacilitatedischarge, which includescommunicatingoptions fordischarge medications andlong-term medication delivery with the patient and/or caregivers,obtaining credit card information andfacilitating Meds to Beds medicationdelivery,if applicable, and assisting with accessing coverage for medication,and engagingthe appropriate staff such asthe Medication AssistanceProgramif medication access needs are identified orcase management ifa prior authorization is required.

The TOCPharmacyTechnicianpromotes a culture of safety by adhering toall hospital and department rules, regulations and requirementsincluding those outlined in theMedication List Documentation and Reconciliation policy, education videos, and Standard Operating Procedures.The TOCPharmacyTechnician must participate in continuing educational opportunities to meet the annual competency requirements and participate in continual training opportunities provided bythe departmentto develop ongoing skills in other areas of pharmacy.The TOCPharmacyTechnicianacts as a positive role model for allTOC pharmacytechnicians and acts in an encouragingmannerconducive to the hospital, department and administration’s visions and values.

Position Summary

TheTOC Pharmacy Technicianis responsible for obtaininga complete and accurateadmissionor ambulatorymedicationhistoryand allergy information.The TOC Pharmacy Technician will help to streamline and facilitate the discharge medication delivery process whenapplicable.

Minimum Qualifications

  • High School graduate or equivalent. Certified Pharmacy Technician. 2 years of relevant experience required. 4-6 years of relevant experience preferred.
