We learn, grow, and achieve more - in our careers and in our communities.\nJob Description Summary:\n***MUST LIVE IN OHIO OR BE WILLING TO RELOCATE***JOB SUMMARY\nThe person in this position will oversee planning, implementation and evaluation of diabetes self-management program at Mansfield and Shelby hospitals.\nMINIMUM QUALIFICATIONS AND DESIRED ATTRIBUTES\n- Bachelor's degree (Required) (Master's degree preferred)\n- Field of Study: Dietetics, Nursing, Pharmacy, Health Promotion, or Health Education (Required)\n- Experience: 3+ years minimum in the field of diabetes self-management education (Required) (5+ preferred)\n- Live in Ohio (Required)\n- Specialization in diabetes self-management\n- Experience in implementing and maintaining AADE accreditation\n- Experiential preparation or education in program management and sin chronic disease management\n- Experience with community outreach in primary or secondary prevention\nResponsibilities And Duties:\n1.20%:\nMonitor implementation of the American Association of Diabetes Educators AADE National Standards for Diabetes Self-Management. 2.\n10%:\nMaintain Diabetes Education and Self-Management program accreditation.3.\n15%:\nPlan, implement and evaluate continuous quality improvement of Diabetes Self Management and Education processes and program.
