Programming should be person-centered, foster community integration, and focus on skill acquisition and, or retention while remaining compliant to agency, state, and federal requirements. This is a management position that provides supervision to the Direct Support Professionals in the program.\n\nESSENTIAL DUTIES AND RESPONSIBILITIES\nThe duties and responsibilities of the Day Services Supervisor include, but are not limited to the following:\nWorks with the Day Services Team Leader to develop, coordinate and implement program goals.\nThe Day Services Supervisor will provide supervision utilizing the Direct Support Professional Core Competencies, JC Code of Ethics & Empowerment Strategies. Provide performance feedback to direct reports. \nEnsures that program records are up-to-date and current for each person enrolled in the service. Works with Care Coordinators to obtain and share documents to ensure complete and contemporaneous files.\nPrepares for and attends Life Plan and other team meetings, and utilizes the information gathered to develop Staff Action Plans and document meeting minutes. \nProvide role modeling, coaching and training to staff on the implementation of Habilitation Staff Action Plan and service documentation. Maintain records of in-service training and communicate any training needs to the Leadership Team. \nWorks with the DSP SR, Direct Support Professionals (DSP’s) and individuals supported to develop and implement the daily schedule of activities. \nProvide support on the implementation of the Behavior Support Plan in coordination with the Behavior Clinician. \nMonitors the electronic health record (i.e., eVero) to ensure complete, accurate and timely complementation of service delivery as required. \nMonitors service delivery as required to ensure quality and adherence to NADSP Core Competencies, Code of Ethics, Staff Action Plan and Safeguards. Take correction actions as necessary and immediately notify Supervisor of concerns. \nCoordinate with Day Services Team Leader to ensure adequate staff coverage levels at all times. Provides coverage and performs direct support duties as needed.\nWorks with the Day Services Team Leader to interview candidates.\nCreates and maintains a healthy and productive work environment by modeling positive behaviors that teach patience, professionalism, and teamwork. \nWorks with Day Services Team Leader to plan and co-facilitate staff meetings. \nFollow and adhere to all agency and program policy and procedures, including HIPAA, Confidentiality, Accident / Incident Reporting, Corporate Culture Playbook and implementing the Corporate Compliance Plan and related policies and procedures, and reporting facility and/or program needs to the Supervisor. \nCompletes special projects as requested by working independently and, or with other colleagues and staff.\nActively participates in supervision for professional growth and development. \nReports to work promptly and adhere to the assigned schedule. \nComplete all required initial and annual training as well as ongoing training to increase knowledge. \nBuilds and maintains professional relationships with staff and colleagues.\nAttends department and agency meetings as requested.\nMaintains a valid NYS driver’s license in good standing and operates agency vehicles as needed.\nPerform other duties as requested/assigned.\n\nQUALIFICATIONS\nGood oral and written communication skills.\nAbility to read, write, speak and understand English.\nAbility to maintain confidentiality.\nStrong interpersonal, listening, communication and motivational skills.\nGood analytical ability with problem-solving and follow up.\nAbility to work independently and handle multiple tasks simultaneously.\nProficiency in the use of a personal computer and appropriate software and E-Mail system.\nValid driver’s license and the ability to operate an agency vehicle if needed.\n\nEDUCATION and/or EXPERIENCE\nBachelor’s Degree or Registered Nurse in a related field with minimum 1 year supervisory experience in the field.\n OR\nAssociates’ Degree in related field with a minimum 2 years supervisory experience in the field\n OR\nMinimum 5 years experience with related population and minimum 2 years supervisory experience preferred\nWorking knowledge of OPWDD regulations and payment standards as they pertain to Supplemental Day Habilitation, Community Habilitation, and Respite.\n\nPHYSICAL DEMANDS\nAbility to perform the essential physical functions of the job including but not limited to the ability to stoop, lift, kneel, push, pull, reach, stand, walk, hear, listen, and communicate
