This role ensures accurate documentation of resident care, supports reimbursement processes, and collaborates with interdisciplinary teams to promote quality outcomes for residents.\nEssential Duties\nComplete MDS assessments and care area assessments (CAAs) within required timelines.\nEnsure accuracy of resident clinical documentation to support care planning and reimbursement.\nMaintain compliance with CMS, state, and facility guidelines.\nCollaborate with interdisciplinary team members to develop individualized care plans.\nMonitor resident progress and update care plans as needed.\nParticipate in resident care conferences and quality improvement initiatives.\nEnsure MDS submissions meet regulatory requirements for Medicare/Medicaid reimbursement.\nAssist with audits and surveys by providing accurate documentation and reports.\nStay current with regulatory changes impacting MDS and skilled nursing documentation.\nWork closely with nursing staff, therapy, social services, and dietary teams to gather resident information.\nProvide education and guidance to staff on documentation standards and MDS requirements.\nCommunicate effectively with residents and families regarding care planning.\nSupervisory Requirements\nThis role has no supervisory requirements.\nLanguage Skills\nMust be able to read, write, speak, and understand the English language. Ability to read technical procedures. Ability to read and comprehend policy and procedure manuals. Ability to effectively present information and respond to questions from managers and employees. Must be able to relate information concerning a resident’s condition.\nMathematical Skills\nAbility to apply concepts such as fractions, percentages, ratios and proportions to practical situations.\nReasoning Ability\nAbility to solve practical problems and deal with a variety of\nconcrete variables in situations where only limited standardization\nexists.
