The SLP also provides ongoing education, in-services, and training for facility staff.\nEssential Duties & Responsibilities\nEvaluate residents to determine speech, language, cognitive, and swallowing therapy needs, functional limitations, and rehabilitation potential in accordance with physician orders and facility policies.\nDevelop, implement, and update individualized treatment plans to improve communication, cognition, swallowing safety, and functional independence.\nCollaborate with nursing, physicians, residents, families, and interdisciplinary team members regarding treatment progress, goals, and discharge planning.\nAssist the nursing department with training and education of Restorative Aides.\nSupervise Speech-Language Pathology Clinical Fellows (CFY) and students in accordance with applicable state practice regulations.\nParticipate in resident care conferences, Utilization Review meetings, and rehabilitation meetings as needed.\nProvide in-services and educational training programs for facility staff.\nComplete timely, accurate, and compliant documentation in accordance with facility, governmental, and third-party payer requirements.\nMaintain a safe treatment environment by following infection control, safety, and equipment maintenance protocols.\nEducate residents, families, and caregivers on treatment goals, home exercise programs, maintenance programs, communication strategies, aspiration prevention, and swallowing safety.\nRecommend and facilitate the ordering of appropriate durable medical equipment to support residents' swallowing, communication, and cognitive needs.\nContribute to quality assurance, performance improvement initiatives, and rehabilitation program development as assigned.\nEstablish positive and professional relationships with residents, families, staff, and colleagues.\nQualifications\nMaster's degree in Speech-Language Pathology from an accredited program.\nCurrent California Speech-Language Pathologist license.\nCurrent CPR certification, as required.\nExperience in skilled nursing or post-acute rehabilitation preferred.\nStrong clinical assessment, communication, and documentation skills.\nAbility to work independently while collaborating with an interdisciplinary team.\nPassion for providing exceptional, resident-centered care.\nReady to Make a Difference?\nIf you are a dedicated Speech-Language Pathologist looking for a flexible PRN opportunity, we would love to hear from you!\nApply today and join the San Bruno Skilled Nursing team!\nWe are an equal opportunity employer.
