The role of the Re-Entry Service Coordinator is to provide community based supports that are designed to promote the acquisition of life skills and access to resources which will support inclusion in the community and lead to permanent housing.\nAvailable Positions: 1\nLocation: In Person (Rutland County VT)\nSchedule: Full Time (35-40 Hours per week)\nSupervisor: Regional Re-Entry Services Team Lead\nApplication Requirements:\nA Cover Letterand Resumeare required with your application.\nResponsibilities:\nEngage in relationship building/rapport building with clients and provide services with a client centered, strengths-based approach.\n\nProvide one to one support and counseling on life skills, tenancy skills, budgeting skills, housekeeping, skills.\n\nAssist clients in navigating the housing path including housing search and applications, viewing available units, understanding lease agreements and tenant rights and responsibilities, assistance with moving in, etc.\n\nEducate, inform, and advocate for clients regarding available benefits and entitlements (Social Security, Public Assistance, food stamps, Hireability/ICAN, etc.).\n\nMaintain written and computerized records, compile reports and complete other program documentation (including case notes, face sheets, client files, application tracking, petty cash receipts, etc.).\n\nCollect and maintain information related to participant’s housing history including: income and income history, rental history, criminal history and credit history (as these relate to obtaining/maintaining housing).\n\nSupport lease ups, move-in, move-outs and regular inspections pursuant to Housing quality standards between tenant, landlord and housing authority inspector.\n\nAttendance at the annual All Staff and Programmatic Retreats is an expectation of employment with Pathways Vermont, unless excused by the employee’s manager/HR. Retreats are held on workdays, during normal working hours, and are fully paid.\n\nOccasional travel to other counties when needed to support participants and community partners.\n\nOther Duties as assigned\n\nAttendance at the annual All Staff and Programmatic Retreats is an expectation of employment with Pathways Vermont, unless excused by the employee’s manager/HR. Retreats are held on workdays, during normal working hours, and are fully paid.\n\nQualifications:\nAbility to work within a client centered, strengths-based framework. A compassionate understanding of the issues facing persons with mental health challenges or substance use issues, particularly as related to meeting basic needs of food, clothing, shelter and advocacy.\n\nExcellent communication, writing, and computer skills\n\nAbility to structure time, develop work priorities independently, and meet program expectations regarding documenting interactions.\n\nValid driver’s license, a reliable personal vehicle available for work purposes and current automobile insurance required.\n\nAbility to lift and carry 50 pounds.\n\nWillingness to self-reflect, learn, and practice new ways of being in relationships\n\nRisk and discomfort tolerance\n\nContinued employment is contingent upon successful PREA certification and satisfactory AHS Background check.\n\nLived experience of homelessness, mental health or other life challenges considered an asset.\n\nPhysical Requirements:\nAbility to lift and carry 50 pounds.\n\nOccasional exposure to outside environmental conditions for community-based meetings.\n\nOften meeting with persons in the community and/or visiting different businesses\n\nExtending hand(s) and arm(s) in any direction at times when collecting paperwork, assisting with applications, etc.\n\nExtensive reading responsibilities\n\nAbility to ascend and descend stairs occasionally required (in instances when businesses and/or meeting places do not have an elevator)\n\nRegular operation of a computer and other office machinery including frequent typing.\n\nGrasping.
