The Care Coordinator has an overall responsibility for coordinating and advocating for all aspects of the individual’s care including, but not limited to, health and behavioral healthcare, community supports, and other services required to meet the needs of the individual. Care Coordinators use a person-centered, holistic approach, where each person being served has their health care and other service providers collaborate to ensure their wants and needs are met. We’re in the business of helping people and always using a strengths-based approach.\n\nSocial workers, care managers, and case managers are encouraged to apply!\n\nPosition Responsibilities:\nDevelops a comprehensive, person-centered Life Plan with the individual and their circle of support, as well as their entire service provider team.\nCommits to a respectful, just, and supportive environment for individuals and coworkers aligning with the company’s commitment to diversity, equity, and inclusion.\nEffectively manage a tiered caseload, while tailoring services to individual needs.\nCompletes enrollment and eligibility documentation.\nMeets with individuals in their homes, physician/provider offices, and other public places in order to conduct assessments and provide services.\nWork independently from home, as well as reporting to the office at least once per week but may require more depending on business needs. \nRequirements:\nBachelor's degree with 2 years of relevant experience OR a Licensed Registered Nurse with 2 years relevant experience OR Master's degree with 1 year of relevant experience\nRelevant experience consists of providing direct care or support, coordination, and/or advocacy for any person(s) on a regular basis.
