Wollongong Private Hospital is a 171 bed private hospital. It features state of the art equipment, facilities and patient services setting a benchmark for healthcare facilities in the Illawarra. Wollongong Private Hospital provides all major services including an intensive care unit, ten operating theatres, cardiac and vascular Labs and an Endoscopy suite.

We are seeking a hybrid Social Worker/Discharge Planner to work with our team at Wollongong Private Hospital.

This is a casual role offering flexible days and hours, suitable to the needs of the service and candidate.

About the Role

As a member of the Allied Health team, you will be working in a key role that provides clinical support to patients, nursing, medical and allied health providers. You will be responsible for supporting an effective discharge planning service and coordinating discharge services across the hospital. You will deliver personal counselling, helping individuals and carers adjust to their current medico-psycho-social situation.

Your role will involve, but is not limited to;

• Work providing social work services including counselling and community follow ups
• Coordinating the patient discharge planning process
• Ensuring patients have a smooth transition from the hospital to the community with appropriate care and services
• Planning complex discharges with the multidisciplinary team

About You

To succeed in this role, you will bring a strong clinical skillset with experience working in an acute care hospital setting. Your effective interpersonal and communication skills (both written and verbal), ability to work in a multidisciplinary, fast-paced team environment, client-focused attitude and willingness to accept and promote Ramsay’s core values are essential for this role.

You will also demonstrate:

• Degree in Social Work or equivalent
• Eligibility for membership of the Australian Association of Social Workers or equivalent Professional Association
• Minimum of 2 years relevant clinical experience in an acute health care setting
• Extensive knowledge of community services and other resources relevant to this role
• Knowledge of and ability to implement contemporary discharge planning practices
• Demonstrated ability to effectively and efficiently manage a caseload
• Excellent interpersonal and communication skills
• High standard of customer service and a team approach
• Commitment to professional development and maintenance of current evidence-based practice
• Flexible and positive approach to work
• Experience with My Aged Care, the residential aged care process, palliative care and maternity patients will be highly regarded

Remuneration

On offer is a base hourly rate ranging from $38.26 - $55.42 + superannuation based on experience level and qualifications, exclusive of penalties or allowances. (NSW Health Professional Level 1-Level 2 gr1-gr4)
