Service Coordinator II and Senior positions also provide leadership for assigned staff, programs, or partnerships.\nCatholic Charities Senior Communities develops and operates affordable, supportive communities for older adults with a resolve to nurture a spirit of purpose, wellness, and harmony among both our residents and colleagues. We offer 24 locations in Anne Arundel, Baltimore, Garrett County and Harford Counties and Baltimore City.\nThe work schedule is Monday - Friday 8:00 AM - 4 PM\nThis position will work between three locations:\nTuesdays and Thursday at Everall Gardens located at 6100 Everall Avenue, Overlea, MD 21206\nMondays and Wednesdays at Kessler Park located at 4230 Hollins Ferry Road, Lansdowne, MD 21227\nFridays at St. Mark's located at 19 Winters Lane, Catonsville, MD 21228\nJOB DUTIES & RESPONSIBILITIES:\nDevelops personalized service plans that identify residents’ needs, connect them to appropriate resources, and support their overall well-being.\nBuilds collaborative relationships with community partners to expand access to services, support resident integration, and improve outcomes for those\nPromotes resident engagement, social connection, and self-advocacy to support well-being and create a more supportive living\nResponds promptly to resident needs and safety concerns, coordinating appropriate supports and interventions to address immediate issues and long-term care planning.\nSupports positive communication during resident conflicts by helping all parties feel heard, respected, and focused on resolution.\nAssists residents, caregivers, family members, colleagues, and community members by providing support and resource information as needed.\nDocuments daily all resident communications and service coordination activities accurately to support continuity of care, accountability, and effective follow-up.\nConnects residents with essential resources and information to support informed decision-making, independence, and community engagement.\nSupports a strong volunteer program that increases community involvement, strengthens support networks, and helps meet resident and community\nPromotes resident awareness and engagement, empowering residents to maintain independence, well-being, and safety in their homes.\nCompletes required training and applies HUD guidelines and standards to support residents and maintain\nCoordinates with community partners to connect residents with care, services, and supports that promote well-being and quality of life.\nCollaborates with Property Manager(s) and other team members to serve residents and identify residents who may need assistance.\nPerforms other duties as assigned.\nPOSITION SPECIFIC DUTIES:\nEmployees working in the following program(s) or position(s) have responsibilities that are in addition to the above core duties and responsibilities. These include, but are not limited to: \nService Coordinator (II and Senior)\nManages and directs the work of one to three employees, including recruiting new hires, onboarding new employees, conducting performance reviews, and supporting accountability and effective team performance.\nEDUCATION & EXPERIENCE REQUIREMENTS:\nBachelor’s degree in social work, human services, gerontology, psychology, sociology, or a related field.\nTwo years’ experience providing social services or service coordination, including assessing individual needs, developing service plans, connecting individuals with community resources and public benefits, maintaining accurate documentation, and collaborating with families and service providers.\nDemonstrated working knowledge of local services for the elderly and people with disabilities.\nAn equivalent combination of relevant education, training, and experience that demonstrates the ability to perform the essential functions of the position may be considered.\nREQUIRED SKILLS & ABILITIES:\nStrong verbal and written communication skills, including active listening and the ability to convey information clearly and respectfully to residents, families, colleagues, and community partners.\nService-oriented and responsive, with empathy, patience, and sound judgment needed to understand concerns and support residents in identifying appropriate solutions and resources.\nEffective conflict-resolution and problem-solving skills, with the ability to remain calm, consider differing perspectives, and support respectful, practical resolutions.\nProfessional discretion, integrity, and respect for confidentiality when handling sensitive resident, family, and organizational information.\nStrong planning and organizational skills, with careful attention to detail and the ability to prioritize multiple needs, maintain accurate information, and follow through in a timely manner.\nCollaborative and relationship-focused, with the ability to build trust, engage others, and influence coordinated action among residents, families, colleagues, and community partners.\nRequires proficiency in using computer systems and software to perform job function, including but not limited to, basic Windows PC, web browsing (i.e., Chrome, Internet Explorer, etc.), and Microsoft Outlook.
