This position is ideal for a behavioral health professional who is passionate about serving adults with mental health and co-occurring disorders while gaining experience with a diverse range of behavioral health conditions in a collaborative and professional environment.\n\nPosition Summary\nThe Clinical Service Coordinator is responsible for conducting comprehensive behavioral health assessments, treatment planning, care coordination, and discharge planning for adults receiving outpatient behavioral health services. Services may be provided in person or through telehealth platforms.\n\nDuties & Responsibilities:\nConduct and monitor biopsychosocial and behavioral health assessments.\nDevelop, implement, and monitor individualized treatment and discharge plans.\nAssess client appropriateness for outpatient groups and services, including referrals from:\nNGRI/IST programs\nProbation and parole agencies\nRegional referral sources\nHospitals and internal programs\nCollaborate with treatment teams and group facilitators to develop effective interventions and treatment strategies.\nCoordinate client transitions and discharge planning to ensure continuity of care.\nMaintain accurate and timely clinical documentation, including updates to client records, releases of information, and treatment plans.\nCommunicate with community partners, personal care homes, courts, probation/parole officers, treatment teams, and other stakeholders as needed.\nPrepare reports and participate in monthly NGRI/IST meetings.\nAttend Supported Employment meetings and provide updates regarding shared clients.\nMaintain a minimum productivity standard of 60 billable service hours per month.\nConnect clients and families with community resources including housing, financial assistance, legal aid, education, employment, and other recovery supports.\n\nQualifications:\nMaster’s degree in social work, Psychology, Counseling, or a related Behavioral Health field.\nEligible for Georgia licensure as one of the following: LMSW/LCSW, APC/LPC, LAMFT/LMFT\nMust actively pursue
