administrative duties to include referral processing, patient registration, verification of benefits, insurance authorizations, patient appointment scheduling, communication with referral sources and other tasks assigned.\nThe Care Coordinator displays an elevated level of professionalism and engages resources and fellow team members within the center to deliver high level results.\nWhat you'll do\nPRIMARY RESPONSIBILITIES:\nReviews referral requests generated by primary care physicians and onsite specialists\nCoordinates appointment date and time for patient. \nInforms patient of appointment date and time, special test preparations as necessary, and schedules transportation when necessary.\nSecures patient information and maintains patient confidential health information.\nReviews open referral report (pending consult and diagnostic) daily, to ensure reports have been received prior to patient's visit with PCP. \nProcess a minimum of twenty referral orders per day.\nProcess all referral authorization per the health plans protocol and systems i.e., Availity\nCoordinates procedures required for surgical clearance and ensures complete documentation is forwarded to specialist's office, ambulatory center, hospital, etc. \nSchedule urgent referral appointment per the request of the Primary Care Physician, or onsite specialist.\nInforms Primary Care Physician via appropriate documentation in the EHR of any missed appointments and reschedule appointments as needed.\nScan and index documents into the EHR; ensure that necessary documents I.e., consult notes etc. are available in advance of patient visits with primary care physicians or onsite specialists.\nParticipates in center and market interdisciplinary teams related to patient care plans, and outcomes.\nAid coworkers as needed to assure smooth office operation and delivery of excellent service through teamwork.\nPerform other duties as assigned \nQualifications\nREQUIRED QUALIFICATIONS:\nHigh School Diploma/GED \nOffice Administrative/Receptionist/Clerical/Customer Service experience in a fast-paced, heavy phones environment.\nProfessional demeanor and communication always.\nMust be organized and attentive to detail. \nAbility to manage competing priorities. \nResourcefulness in problem solving\nAble to take and follow through with assigned tasks and accountability.\nAbility to work in a fast-paced environment\nExperience with Microsoft Office Word, Outlook, and Excel.\nPREFERRED QUALIFICATIONS:\nExperience working with an electronic health record \nExperience in processing referral authorizations\nExperience with Availity or other referral authorization platforms\nExperienced working in medically underserved/culturally diverse
