They are responsible to lead shifts - guiding and motivating the team to achieve organizational goals.\n ACCOUNTABILITIES:\nThe incumbent may perform all, some, or similar duties, those listed below:\nFrontline Direct Client Support\nProvide inclusive supports which can include treatment, or rehabilitation plans for individuals within the assigned Lawson Ministries programs\nAssist individuals in developing person-directed goals; helping track and monitor progress in conjunction with staff, family, and others involved in the individual’s life\nProvide personal care to individuals as required\nAssist with supporting individuals’ day-to-day living responsivities (i.e. household chores, meal prep, etc.)\nAdminister, accurately document, and order medications for supported individuals\nCoordinate community-based activities and natural supports to foster community engagement\nNavigate community resources to identify opportunities for service users to build skills and realize community-based goals\nLeadership of Team Culture\nAssist with coordinating day-of staffing assignments based on available staffing resources and client demands\nCheck-in and communicate with team members as needed during the course of their shift; documenting any required action items\nCompletes daily on-call reports for the shift worked\nAssist with any type of emergencies that may arise during the shift; and provide assistance to other programs as needed\nMonitor level of program supplies; requesting re-order/purchase as needed\nEnsure the AIMS Case Management software is used for all documentation and support team members in using the software\nScheduling\nEnsure program coverage in accordance with government, contractual and organizational requirements for assigned program\nComplete scheduling in accordance with call down practices; resolving scheduling conflicts as they arise, and liaising with 3rd party staffing agency to coordinate substitutes as needed\nInput requests from employees for time off including vacation requests in accordance with organizational policies\nDistribute weekly schedules for assigned program\nCoordinate with Supervisors and Administrative staff to schedule annual trainings for employees\nQuality Assurance Measures (QAM)\nComplete physical site spot checks; ensuring proper documentation\nComplete bi-weekly QAM checks for assigned location; follow up with Supervisors or Administration as required\nEnsures front line staff maintain the general upkeep, cleanliness and maintenance of applicable programs spaces\nPerform other duties as required\nCRITICAL RELATIONSHIP MANAGEMENT\nInternal:\nDirectors, Supervisors, Co-workers, Front Line Staff, Volunteers & Clients External:\nCommunity Members, Vendors, Client’s Family Members\nMANAGERIAL RESPONSIBILITY:\nReports directly to the Program Director or designate\nThis position has no direct reports\nFINANCIAL AND MATERIALS MANAGEMENT:\nUses or maintains organization assets in accordance with establish guidelines\nUtilizes assigned equipment and supplies appropriately as per policies and procedures\n WORKING CONDITIONS:\nThis is a full-time position based on 42 hours per week\nSchedule may vary according to requirements of responsibilities; weekend and evening work will be required\nHours of work and work schedule will be set in coordination with the employee’s supervisor\nThis position is not considered a management position for the purposes of the Employment Standards Act of Ontario\nRegular travel between locations in the city may be a requirement of this position (within Hamilton, and surrounding area)\nPosition will be in-person due to the requirements of the position\nPhysical lifting of us to 20lbs may be required\nThe above responsibilities must be discharged in accordance with The Salvation Army’s Mission Statement, in a professional manner, upholding our code of conduct.\n EDUCATION, EXPERIENCE, AND QUALIFICATIONS:\nEducation, Qualifications and Certifications:\nCompletion of a formal post-secondary/college program of two academic years: Developmental Social Worker Diploma or equivalent in Human Services field\nCurrent First Aid, Cardiopulmonary Resuscitation with Defibrillation Training, and current Non-Violent Crisis Intervention certification are required\nAlternative combinations of education and experience may be considered.\nExperience and Knowledge:\nMinimum of one (1) year of prior related experience working with adults with developmental disabilities and vulnerable populations is essential\nLifts and Transfer training and Personal Care Support experience are both considered an asset\nProficient in working in a computerized environment; must demonstrate strong skills using Microsoft Office Suite: Outlook, Word, Excel, Power Point.\nSkills and Capabilities:\nAct as a role model for inclusive, respectful and ethical practice\nSelf-motivated/disciplined\nAbility to maintain information in confidence and exercise good judgement\nDemonstrated ability to work independently and participate as an active and responsible team member in a cooperative team environment\nExcellent interpersonal skills, integrity, and adaptability\nAbility to handle complaints and resolve conflicts\nExhibit good listening skills, have strong oral/written communication skills and respect authority\nParticipate in mandatory orientation training, on-going professional development and training\nParticipate in regular supervision and performance appraisal processes\nDemonstrate an understanding of the psychological and social dynamics of working with individuals who experience complex, responsive behaviours.\nWork in compliance with OH&S Act and Regulations and abide by The Salvation Army’s health and safety policies and procedures.\nRepresent the organization in a positive, professional, and engaging manner.\nComply with all Salvation Army policies, procedures, and associated legislation.\nTreat the property of The Salvation Army with due care and caution.\nMust have at minimum a valid Ontario Class “G2” Driver’s License, personal vehicle an asset, current copy of driver’s abstract that is satisfactory to The Salvation Army, in its sole discretion, is required\nAbility to provide an original copy of a Background Check with vulnerable sector screening that is satisfactory to The Salvation Army, in its sole discretion.\nWillingness to develop an understanding and support for the mission and purpose of The Salvation Army in Canada and its implications as related to position responsibilities.\nCompensation:\nThe target hiring range for this position is $50,897.60 to $63,627.20 with a maximum of $76,356.80.Placement in the salary range will be based on factors such as market conditions, internal equity, and candidate experience, skills, and qualifications relevant to the role.\nThe Salvation Army will provide reasonable accommodation upon request.
