Contacts referring provider office for new order if incomplete.\nManages work queue for nutrition and diabetes services for multiple locations throughout the system\nFollows scheduling policy and procedures to accurately identify patient, insurances, referrals, and obtains required signatures where applicable\nAccurately screens ABNs to determine medical necessity, explains ABN to patient, obtains signatures and reviews document for completeness.\nProvides basic nutrition and diabetes specific health information\nGenerates reports to identify cancellations/no shows and reviews service history to determine need for rescheduling\nFacilitates collection of data for CQI & KPI initiatives\nObtains appropriate referrals for outpatient follow-up services post-discharge from hospital\nConnects with patients post-discharge from hospital to complete patient experience survey\nCommunicates with referring providers for any patient not receiving services as prescribed (ie unable to contact, no-show, multiple cancellations, etc)\nGenerates daily reports for inpatient glycemic data\nAssists with data collection for annual accreditation reporting\nSchedules conference rooms for group session, staff meetings, trainings, etc\nAssists with management of staff outlook calendars\nGeneral clerical duties as assigned\n\nCompensation (Commensurate with
