This role is ideal for a qualified healthcare professional who has transitioned into healthcare administration, care coordination, or virtual medical support.\n\nThe Clinical Care Coordinator will serve as an important link between patients, providers, insurance companies, and other members of the care team, helping ensure patients receive timely and well-coordinated care.\n\n Responsibilities\n\nCommunicate with patients regarding appointments, treatment plans, follow-ups, referrals, and other care needs.\n\nCoordinate patient care between providers, specialists, facilities, pharmacies, and other healthcare professionals.\n\nAssist with patient intake and maintain accurate patient information within the EHR/EMR.\n\nDocument patient interactions, clinical information, and provider instructions accurately.\n\nCoordinate and follow up on referrals and specialist appointments.\n\nAssist with insurance verification, eligibility checks, and prior authorizations.\n\nFollow up on outstanding medical records, laboratory results, imaging, referrals, and other clinical documentation.\n\nConduct patient follow-up and care management calls, including Chronic Care Management (CCM) calls when required.\n\nReview patient information and identify issues that require escalation to the appropriate clinical provider.\n\nCommunicate provider notes, instructions, and other relevant information to patients.\n\nSupport prescription/refill requests and other clinical administrative workflows as appropriate.\n\nMaintain accurate, confidential, and HIPAA-compliant patient records.\n\nProvide compassionate and professional patient support while helping patients navigate their care journey.\n\nQualifications\n\nClinical background required; candidates with training or professional experience as a Nurse, Doctor, Physician Assistant, Medical Assistant, or other healthcare professional are encouraged to apply.\n\nStrong understanding of medical terminology and clinical workflows.\n\nPrevious experience in care coordination, patient coordination, medical virtual assistance, clinical administration, or a similar healthcare role is strongly preferred.\n\nExperience supporting U.S.-based healthcare practices is highly preferred.\n\nFamiliarity with EHR/EMR systems.\n\nExperience with insurance verification, prior authorizations, referrals, or care management is an advantage.\n\nStrong English communication skills;
