Southbridge Ottawa is proud of our new state-of-the-art home and welcomes you to join our team!\n\nReporting to the Director of Care, the RAI Coordinator will include duties such as: \nCoordinating the completion of MDS assessment on all residents within established timelines\nUnderstanding how the MDS assessment integrates with existing clinical tools and provides leadership in streamlining these processes\nUnderstanding and providing critical analysis of data at different levels of aggregation (Resident, Home Health Zone, etc.)\nArticulating for staff the strengths of MDS as a clinical assessment tool \nArticulating the financial implications of coding inconsistencies\nActing as key contact and liaison with: Health Zone/AH&W/CIHI/PCC, etc. \nDeveloping facility specific material within the PCC program as required\nResponding to Health Zone on all submission errors\nFacilitating response and correction of submission errors with facility staff\nCoaching to key facility personnel to build capacity for understanding RUG patient classification levels\nDeveloping and introducing processes, and systems to improve accuracy of RAI content\nFacilitate audits of RAI assessments for content accuracy\nParticipates in strategic planning for program development/improvement\nResponsible for Clinical and Admin Setup role to support MDS within PCC software program\nInterprets reports generated by Health Zone/AH&W or CIHI and provides critical analysis for Home\nUnderstands the development of RAP’s, quality indicators, and RUG case mix products\nThe qualified candidate will have:\n3-5 years experience in Long Term Care, experience with MDS an asset\nRegistered as a RPN under CNO\nKnowledge in nursing rehabilitation and working OT and PT\nStrong analytical and assessment skills\nAbility to lead training and education with Adult learners\nKnowledge of: InterRAI suite of tools, MDS/RUG III patient classification systems. \nEffective communication skills \n We would like to thank all applicants for your interest in working as part of our team!
